Totally Stopped hair loss For 15 Years. Now Loosing Like Crazy. Why?

Hello.

I can't figure out what's hapening with my situation, maybe some of you might get me on the right tracks or make me ask myself the right questions because I'm kinda lost now...

Here's the situation :

I've been an early hairloss sufferer. I started to loose ground pretty hard at age 20. But after a few years of research on forums, I started the big 3 (dutasteride, minoxidil 5 and nizoral).
It did eventually completely stop my hairloss. At a point that I totally forgot about hairloss forums for 15 years and never thought again about my hair until... 15 years later...
Now, aged 39, since july 2019, I've been loosing hair like crazy and my hairline is receeding like crazy. I've lost about 3 cm of hairline in one year...

I've been to the doctor, done some tests, everything seems fine.
I have very low DHT (which is expected with dutasteride) and Testosterone just a bit over the limit.
I don't know how these hormones were before as it's my first blood test.
I had low B9 so I'm now supplementing with B9 (folic acid) but it doesn't seem to change anything.
Other vitamins, thyroid, are ok.

I've been to the dermatologist which is supposed to be specialised in hair but he kinda said to me he can't really do anything. Appart from prescribing finasteride and minoxidil he doesn't seem to be able to do much...

The only thing that started around the same time is that I started to have massive hives, which is treated by cetirizine successfully (but whenever I stop the hives comes back). The cause, like in the vast majority of chronic hives, is unknown. I've been checking on allergies but all my test were negative.

I'm having a really hard time beleving it's the treatments that "stopped working". As my loss has been really sudden and massive.
I kinda hoped it was a telogen effluvium or something temporary but it is clearly not...

I've started dermapen since 6 months but it didn't change anything.
I'm gonna try zix and topical cetirizine but I have moderate confidence in these treatments...

Maybe what caused the chronic hives is also causing the hairloss but I can't be sure and moreover diagnosing the cause of hyves is almost always a fail...

Do you guys have an idea on the possible cause, or a direction I should look into?

How very curious. I'm so sorry for you to have suffered a dramatic change after so much success.

I've been on Minoxodil for 16 years, or on Finasteride for 13 years, but I've not really seen much further hair loss since. I noticed a slight deterioration about 3 years ago, but then I changed my Minoxidil to a formula of 8% together a topical female hormone called Medroxyprogesterone (MPG) (from the Belgravia Clinic in London) and it's stabilised again.

I'm of the same opinion as you that it's not credible that the medications have stopped working and that there must be something else at play here. Have you tried stopping all medication for say a month and then restarted? That'd be my first option.

All I can suggest is that you contact Belgravia Clinic in London and ask them if they can diagnose it. They patent the 8% Minoxidil plus Medroxyprogesterone, and they also patent a 12% Minoxidil which is very powerful.

Many people forget testosterone also damages the follicle, not only DHT. And fina doesn't block all DHT too...

Yeah while much weaker than DHT over the long term T can still damage hair, especially considering the increase of scalp T when using dht blockers.

15 years is a really good time. i think there isnt much you can do now if you are already on duta.

you may try out things like duro but i dont know if its worth it.

all in all your case is still a success story i think

I don't think testosterone is so terrible. If testosterone really affects you, you won't be able to spend 15 years successfully. I think this thing is related to your immune dysfunction, although there seems to be little evidence. If you can understand Chinese, please look at my photos, which may give you some inspiration.

Take a look at the cases of minoxidil that, after several years of successful use of minoxidil, caused scalp inflammation and hair loss. Maybe these examples can tell you something.

It would take a lot of testosterone.

To date, baldness has not been noted in any male pseudohermaphrodites with 5a-reductase deficiency from anywhere in the world.

This also means it's not immune dysfunction. That is alopecia areata. It's totally different.

All depends on how sensitive your hair follicles are to androgens

If you have very sensitive AR, and the balding process is already in full swing, with hundreds of genes already differentially expressed then I could see testosterone having an effect. For most people though, once DHT is out of the picture testosterone isn't potent enough to do any damage.

Do you then think some people continue to lose ground on dutasteride because of the remaining 49% or so of DHT left on their scalp and T isn't the culprit?

Right. Few people continue to lose ground on dutasteride. For those unlucky people it's just that it doesn't reduce DHT enough. If you could completely block DHT it would be very rare for Androgenetic Alopecia to progress. Most of those would probably be cases of mutant AR.

Btw i just read your regimen, why such a low dose of spironolactone? 100mg only has 30% chance of gyno, that probably goes up a bit in conjunction with dutasteride though. I thought 25mg would only be effective as a diuretic and wouldn't really do enough to tackle hair loss, you are very well informed though so im probably wrong lol
