Medical castration is temporary and nowhere near physical castration, if you stop hrt you will lose hair again. speaking from my experience I came off hrt 1 month ago and my testosterone is getting tanked. In adri case he will never go bald.

Im still not 100% physically castrated. I am on the waiting list, but tbh it is basically the same since my balls are ischemic, doctors couldnt detect blood flow on the doppler ultrasound and they are hard as rock so yeah very useless at this point
