spironolactone topically works - as quoted by Dr Lees "(1) inhibits the synthesis of DHT, (2) converts testosterone in the scalp to estrogen, and (3) blocks the androgen receptor sites on the hair follicles, effectively preventing even circulating DHT from attaching to the receptor sites."

There's conflicting information as to whether or not spironolactone acts as a 5a-reductase inhibitor. I've seen one study which said that it does do that, and another study which says that it doesn't.
