I was reading on here earlier that finasteride won't work without nizoral, although I thought that couldn't be true
finasteride is one of the few well-documented drugs.
finasteride, on its own, will (on average) buy you five years. As in, five years from now you will be where you are now. In two years, you will have maximum benefit.
finasteride inhibits one of the five-alpha reductase molecules in the body, leading to a ~70% reduction in dihidrotestosterone (DHT), in turn leading to less attack on your hair follicles. The reduction in blood DHT leads to a reduction in scalp DHT.
Nizoral does stuff to the scalp. It's not clear what I think it reduces acne, maybe reduces scalp levels of DHT, not sure.
If finasteride on it's own would buy me 5 years, what would the big 3 together buy me? I've already been using nizoral on its own but has had no effect on the hair loss or itchiness. Thanks for your replies
Note that five years is the average, some do worse, some do better.
The question you ask isn't answered by the literature as far as I know.

which says that finasteride + 2% minoxidil > finasteride + 2% ketoconazole > finasteride > 2% minoxidil
But it's a 1-year study, and they only have 100 patients. The one group taking ketoconazole shampoo only included 10 patients, so the impact is probably not statistically significant.

What a lot of people on this forum are doing, I think, is switching to duasteride, which is like finasteride but a lot stronger, it reduces DHT by more than 90% rather than 70%, however there are fewer long-term studies so there may be more risk involved.
I personally plan on starting finasteride in a few months. My hope is that in five years, there will be other, superior treatments available.
I wouldn't count on better treatments being available within 5 years, I've seen posts on here from 2003 saying they're waiting for a better treatment and almost 16 years later there's nothing

This is why no matter what treatment regimen you use, Nizoral shampoo absolutely must be included. Though it is marketed as an anti-dandruff shampoo, it effectively handles both the hormonal and inflammation-related causes of hair loss. It is the only shampoo that can do this.

oh yeah and finasteride doesn't lose its effectiveness after 5 years, that's a load of crap made up from David countless people on here have been on it for over 10 years and still the same.
I never said it stops being effective after five years -- work on your reading skills.
The effectiveness declines over time, and it buys you approximately five years.

The yellow line looks like it will return to baseline ~7 years after starting treatment, and the red line is back to baseline 4 years after starting treatment. The people in the yellow line sample started earlier, so they probably bought more time, either that or it's statistical noise.
After those five years, the patients look to maintain the spread over where they would be without the drug ... but they will still be losing over where they once were.
