It is proven that both type 1 & 2 of the enzyme 5-Alpha Reductase (5-AR) converts testosterone in men into DHT which is the culprit of male pattern baldness (male pattern baldness.) I have heard that some medications such as minoxidil and propecia blocks off type 2 of this enzyme successfully for the treatment of the vertex part only and that the anterior (frontal) is poorly restored. If it is true that some medications blocks off both types 1 & 2 and are successful for the treatement of both anterior and the vertex, So if the 5-AR enzyme that is converted to DHT and that DHT alone causes male pattern baldness for both anterior and the vertex, then is it logical to think that there are also 2 types of DHT? As far as anyone knows, minoxidil has no effect on DHT, the most important, but not the only, androgen in pattern hair loss. Also, medical treatment works in front, just not as well as in the back. There are two different 5-AR, but it appears that the Type-2 enzyme is not of much importance in balding. Peter H Proctor, PhD,MD
