Age : 42 yrs

Hair loss started at age of 20 .. so it's been slow gradual process for 20 years. On average losing 50-75 hairs every day for past 20 years. Temples gave up in mid 20s.

Hair texture is very thick all over head .

Not on finasteride or Min.

Recently dermatologist said that my ferritin level is in 50 and not 70 so hair may not be growing back also I am vitamin D deficient, which could be playing part with hair not growing back on hairline.

Per Doctor, I have slight receding hairline and he recommended not to get on any medications.Per Doctor, I can't get on finasteride because of past hormonal issues in teen. Also my Scalp is very sensitive to any oil/gel or even most of shampoos so he is not sure my scalp would be able to tolerate Min.

Here are few question i have.

Is this just Matured hair line or male pattern baldness?

Am I NW2 or NW3?

Trying to decide if i should go with hairline transplant (without medication) or wait for future treatments like Brotzua lotion, HSC or follicle.

In case of hair transplant how may grafts would i need to get temples filled and fix hairline( no lowering )?

I think there might be some very minor recession. But generally your hair is very good and doesn't seem to be thinning elsewhere. So at your age you can expect limited loss in the future most likely. Do you normally comb your hair back like that, maybe some traction alopecia instead of male pattern baldness (male pattern baldness).

Shame you can't take finasteride. It would be the first logical step to prevent further loss if there is any. I wouldn't bother with Minoxidil due to the hassle and the fact you don't know any serious issues.

Too early for a transplant although such things are possible for someone like you given your age and generally having good hair.

My recommendation: have a miniaturisation map test which will confirm male pattern baldness if it's there. Look it up. After that you will have an idea what to do.

Thanks WolfPack for your detail response.

I do not comb my hair back so it is not due to traction. I pulled it back for pictures. My left side temple has not receded much but hair line is going in V shape on that side. On Right side temple has receded more than left but hair line is holding pretty well. I keep my hair on side normally but due to right side hair line receding it is getting difficult to continue with that style. I guess i may have to change my hair style after more than 15 years.

I have started noticing hair line receding in last 2 years without any shedding.

Any guess with how many grafts would it take to get hairline fixed and fill temples ?

That's your hair at age 42? Bruh, chill. You don't need anything. Just enjoy life and forget about this sh*t.
