I am a 24 year old male with no history of aggressive hair loss in my family. Half Caucasian half asian descent. When my corners started receding at 22 i decided to start finasteride. I dont remember paying attention to hair shedding before but I can say that the shedding has definitely not decreased maybe increased since being on Finasteride for a year. The scariest part is about 2-3 months ago I started seeing way more miniaturized hairs that were shedding. I dont remember seeing so many miniaturized hairs shedding before. Maybe the odd one but not so much. This morning I saw 3 miniaturized hairs on my pillow. I also see a ton of long terminal hairs shedding as well and they shed from the non DHT zones as well. I am confused about what to do. Is this a sign that my hair loss is getting agressive. I am guessing probably so. Also, finasteride maintained or maybe even possibly made my right temple better however the left side has continued to recede on Finasteride so I dont know if it is working. I dont know what to do. Majority of the men in my family are norwood 1s or had hair loss way later in life so I am feeling very unlucky. Does anyone know what this shedding means and how to stop it? Thank you very much.

I attribute this to the fact that we have asymmetry. If we mentally divide the prostate gland into two lobes, there will inevitably be asymmetry. For example, the left side will be larger than the right. And since the prostate gland is androgen-dependent, testosterone passes through it and is converted into DHT. Accordingly, the left side will be balder than the right, since in your case, the left is balder than the right. In my case, this is exactly the case! Before HRT, I felt inflammation in the left lobe of the prostate. After HRT, I no longer feel this inflammation, and new hair has appeared on my bald left spot, although none had grown there before!

Take a look. The left side is actually thinning more than the right. Photo 1 is my left side, photo 2 is my right side. In photo 1, the hair is short and thinning. In photo 2, there is still some long hair.

Yes it does seem to be a common phenomenon. However it is interesting that the finasteride hasnt been able to stop the recession. my left temple was actually less receded however it quickly receded a couple of months ago making it more symmetrical with the right temple. What do you think about the shedding situation that I wrote about?

Finasteride is a controversial drug; it helps some people, but worsens hair loss in others.

The only thing that undoubtedly helps is the complete cessation of testosterone production with cyproterone acetate (androcur) and the replacement of the main male hormone testosterone with estradiol.

That is insane though. i think that is a very extreme method of treatment. also just out of curiosity how would finasteride make hair loss worse when it at least somewhat surpresses DHT.

Many people on this forum have written that finasteride, if used for a long time, could even worsen hair loss, if you delve into the forum's history, so they sought more extreme methods of hair restoration.

After 13 months of finasteride and 10 months of minoxidil, I can confidently say that my hair has receded by half an inch and continues to fall out at my temples. My right temple has always been much worse than my left, but, remarkably, over the past two months, my left temple has caught up and surpassed the right. It's gotten to the point where Nanogen and Dermatch are no longer effective in concealing the diffuse hair loss at my temples.

Ever since I started finasteride 1 year and 5 months ago I am easily able to pull hair out of my head from both the sides and the top. It started about the 3rd month in and really has never stopped.

just hit 6 month mark on monday...dont feel the shedding ever stopped.

might have slowed down for a while but has flared up again, if it ever slowed down.

when i shower and run my hands through my har atleast 7 hairs come out...and can repeat that quite a few times before it stops.

makes me nervous even though it can take up to 8 months before you see results.

no question it's thinner now than it was 6 months ago.

Its raised testosterone.
Some body think testosterone on follicle is a virus.
Its all about auto immune disease.
Hair loss its auto immune disease.

So, let's break this down to it's components.

You are in a family that doesn't go bald. You somehow do(genetic).

You do aggressively at that. DHT decreasing by 70% isn't helping at all.

Which ultimately means somewhere in Scalp Tension --> Inflammation ---> DHT ---> Fibrosis/Calcification ---> Hypoxia ----> Miniturization cascade, there is something very strong that doesn't depend on much DHT.

I would imagine your scalp is just naturally more hypoxic and damaged from DHT than most. The damage is done. More fibrosis, less subcutaneous fat, blood flow.

You need to ignore DHT as a primary driver. Focus on downstream and upstream factors from it.

Downstream is fibrosis/calcification. Scalp massage, microneedling, GHK-CU/AHK-CU, LLLT. There are so many ways to treat that.

Minoxidil is very good for blood flow but very weird to apply in terms of crust, and texture. Also, the results from it go away after ceasing. The stuff I listed above, are permanent.

Upstream is scalp tension. Your particular headshape in relation to your skin/fat pliability in your scalp. Scalp massage and scalp pumps are the ultimate treatment I've come across so far. That and botox in the galea but that is a whole other level in terms of what you'd probably be comfortable with.

I'm giving you bullet points on about 12 years of research I've done on this. There's so much detail but I'm condensing it to make it easy for you. I would highly recommend researching old threads on this forum.

I'm like you, at the exact same age, finasteride just stopped working. And I refused to go back on 5 mg fincar or dutasteride at all.

I am, however a strong regrower. Not a lot of guys are like this. I can regrow hair anywhere I treat. So I really don't care to much about losing hair. I've been slacking the past year and lost some hairline but I have mature follicles growing like half an inch from where my hairline is. It looks crazy but all that is to say, you should definitely see if you are a strong responder to things like microneedling, AHK-Cu, LLLT, etc.

Failing that, it will be hard to keep your hair.

DHT is an essential part to hairloss but it's not the beginning or the end of it. You can treat it in a lot of ways.

EDIT: I am interested in topical anti-androgens too to be fair.

Thank you very much sir for your reply.

So have you quit using oral DHT blockers completely right now? Do you think in our situation where it is not working it is wise to quit it completely or should we still continue taking them in case it has minimal benefits.

Also what you said it is very interesting to me because i have never considered the effects of things outside of DHT. I do use oral minoxidil 2.5mg. Have you personally tried GHK-CU and have you seen any benefits?

Also my family having no high norwoods, and the men with hair loss having started later and following a slower hair loss projectory mean anything for me or is it completely individual. Btw have you seen my hair loss pictures on my profile and what do you think about it.

Thank you very much brother. Life is hard but hair loss is harder...

A few different issues are being mixed together here. Seeing several short, thin hairs on the pillow does not prove that finasteride is making the hair loss worse. Without trichoscopy, it is difficult to know whether these are genuinely miniaturized hairs, broken shafts, or hairs at a different stage of the growth cycle. Daily shed counts also fluctuate and are not a reliable way to measure treatment response.

One temple changing more than the other does not automatically mean treatment failure. Frontal recession can be asymmetric, and the response at the temple corners is less predictable than at the mid-scalp or crown. The useful comparison would be standardized photographs and trichoscopy from before treatment and now, assessing density and hair-diameter variation.

Since you also mentioned using oral minoxidil 2.5 mg and shedding long hairs from different areas of the scalp, the date you started it, any dose changes, recent illness, rapid weight loss, major stress, scalp symptoms, and other possible causes of diffuse shedding should also be reviewed.

We would not recommend stopping finasteride or oral minoxidil—or adding strong hormonal treatments—based only on forum comments. Some explanations mentioned above, including prostate asymmetry causing one-sided recession, androgenetic alopecia being an autoimmune disease, or finasteride routinely making hair loss worse, are not established conclusions.

A dermatologist should examine the scalp and determine whether the current treatment is providing maintenance, whether another shedding process is occurring, and whether any medication change is appropriate.

We are commenting from a clinic account, so this is general guidance rather than a diagnosis.
