Hi,
I'm 23 and have had hair loss since age 18. The most likely cause of this has been determined as genetic as my father and grandfather had the same male pattern baldness.
I've tried oral finasteride (and minoxidil) which led to side effects within a month hence me discontinuing. I continued with the minoxidil for a year and a half and it was going well but it felt like a losing battle near the end.
Any suggestions, advice or general comments please

would shave it bald. i'm really sorry for you! I am 26 and have hardly any hair left. the bald spots can no longer be overlooked on my scalp.

There are only the "big 3" treatments and I would avoid everything regardless. You can only see the reports here about side effects in RU.
it would only be possible to switch to oral minoxidil or topical finasteride and that together with microneedling.
in addition, only helps to shave. be strong and get help from a psychologist if necessary. it is heartbreaking, but believe me: you are not alone!

I’m sorry but you let it go too far. Only way to regain hair is to prep for a transplant(s). That means following @Norwoody ’s regimen and then scheduling an appointment with a surgeon

I think more people need to see your case. @randolf_faust, you can add this case to your list. You have aggressive male pattern balding. Most people don't realize this is what happens when your gene is aggressive. You could try topical finasteride-minoxidil combo but your case is way too aggressive to reverse.

However you could get away with a no 2 or no 1 guard on the clippers for now.
If it becomes a Norwood 6. Clippers with no guard or wet shave it.

I'm happy with mine..
I never felt happy bald as a cue ball at Norwood 7.

I wouldn't bother with a hair transplant, it's likely to get worse.
And you can't get good results from a hair transplant going worse than a Norwood 4 anyway...
Hair transplant + hair loss meds usually to stop it going Norwood 5,etc...
Hair transplants are super expensive like 30 grand normally.
And the places to get them done cheap such as Turkey can't be done yet with pandemic restrictions on travelling.

Yours looks like Norwood 4 or 5.

Mine without my hair system is a Norwood 7.

You can easily get good results from a NW5 and even a NW6 from transplants. As a NW6 you just need to be willing to do multiples surgeries with at least 1 large FUT for maximum grafts. NW5, still multiple surgeries but you can do FUE with good donor characteristics. This guy is a NW6, you can tell because the top sides are going. Problem for him is that he's 23 and could easily regress to NW7, which is harder for transplants but still not impossible.

To OP, you gotta get on the full treatment stack. You still have hair that can be salvaged. I would try needling + min + topical finasteride to avoid some sides. You're not going to get back a full head of hair but you could see some real improvement. If you can't handle any of that due to sides, I would suggest the shave as well. Hairpiece is always an option but everyone you know will realize it when you go from NW5/6 to perfect hair over night.

As you are a really young guy I would suggest to go for 0.5 mg duta ED and 5 mg oral minoxidil ED. The strongest treatment approved by the specialist. Maybe you can expect some regrowth because youre so young and the miniaturization process could be reversed.
Stay out of the topicals cause you are looking for the best results and that means oral treatment.
