This is only posted in the male section because, though I am a trans woman, my hair loss is the result of male pattern baldness before transition.

I was turning a corner.

My hair loss regimen and hair transplant had not recovered the appearance of a full head of hair, but I had enough back to combine it with a hair system that integrates into my existing hair rather than shaves it all off and kills my scalp with glue.

That hope seems to be all gone, because my hair is rapidly falling out again.

As a test, I just ran my hand gently through my hair for 20 seconds over a white sheet. There are now about 40 hairs on the sheet.

I have been tsking finasteride, dutasteride, topical minoxidil for a year. I was microneedling for four months but stopped that last week in case that was causing the loss. I have been on biotin for two weeks and nizarol for a week.

Most importantly, I have been taking HRT to varying degrees for a year. Mostly in the last three months. Spironolactone 100mg twice daily and a LOT of oestradiol valerate.

My hair is still falling out.

Any suggestions?

I can't even go with the hair system that I was besotted with as a solution to my problems now (if the hair it is attached to is falling out, bye bye system).

Am I totally fucked?

Modern hair systems are not attached to your growing hair; they are attached direct to your scalp (you buzz down any remaining hair underneath) with strong double-sided tape or glue. If you are attaching to your hair you are using old-fashioned technology, which will ultimately cause traction alopecia.

is this legit? I freak out when 3 hairs fall out when I run my fingers through it. 40??? WTF

If you are attaching to your hair you are using old-fashioned technology, which will ultimately cause traction alopecia.

I am not using a hair system yet so that isn't the cause of this recent loss.

That's the type of hair system I was due to start using. Describing it as attached to existing hair was an oversimplification on my part, but traction alopecia is still something I worry about.

Anybody have any ideas whether this recent uptick in loss could be due to something less scary than a resumption of chronic hair loss?

A shed to make way for thicker hairs to grow in? (Oh how I wish it could be that...)

Stress loss due to being outed as trans and exiled by my family at the start of Feb, whilst starting a new job?

Post PRP shed?

I am so scared.

Normally, spironolactone and estradiol leads to hair growth...? Would you try a stronger form of an anti-androgen? Such as Androcur, or something

I'd consider it, sure.

But I can't make sense of this problem.

Finasteride / dutasteride and estradiol seemingly stopped any hair loss (aside from usual turnover) for 11-12 months, even when my testosterone was still quite high or just on the low end of the male range.

My testosterone has been in the low end of a cis woman range for a few months now, since January or so.

I don't why aggressive hair loss could have started in the last month. I have virtually no testosterone, and therefore surely virtually no DHT?

Argh!!

Argh!!

I think it may be reasonable for you to try Androcur, because that will stop your testosterone production (it's a form of chemical castration). It's used by many transgender women. If that doesn't work, then there must be something underlying which is causing your hair loss... There is oral minoxidil, but I wouldn't recommend that since it can lead to bad skin and more facial / body hair which is not desirable for any woman. Do you think it could be an underlying issue?

I have been on 100MG of spironolactone and 2MG of estradiol for just over two weeks and I'm planning to switch to Androcur (cyproterone acetate) because I'd feel more comfortable knowing I have none of the hormone which causes hair loss.

Can you show an image of your hair, potentially?
