I would advise you to use estradiol, it thickens the hair on your head and prevents hair loss.

I've got it too. DHT blockers seem to have stabilised it but I never got any regrowth. There's limited knowledge on retrograde / DUPA but it's believed to be a rare form of androgenic alopecia so you can only treat it in the same way that you would treat regular hair loss. When you see Norwood 7s out and about, they will usually have some thinning on the back and sides too.

From what ive read on people with dupa they’re extremely nonresponsive to dht blockers, there’s people on the dupa subreddit who have gone all the way to 2.5mg dutasteride and oral estradiol and still experienced no regrowth. The working theory now is that its either an autoimmune condition or hyperprolactinoma which is high prolactin in the blood due to pituitary tumors.

I completely understand why this feels overwhelming, but before assuming the worst, I would really try to get a proper diagnosis from a dermatologist or hair-loss specialist who is familiar with diffuse thinning and DUPA.

Diffuse thinning can be especially difficult because it doesn't always follow the typical Norwood pattern, and it can also affect the donor area, which is why a proper assessment is important before considering something like a hair transplant.

And even when regrowth isn't fully achievable, there are still cosmetic options that can sometimes help reduce the visual contrast between the scalp and the existing hair. Scalp micropigmentation, for example, can be used in some cases to create the appearance of greater density without claiming to regrow hair.

But first I would definitely focus on getting the diagnosis confirmed and understanding whether your hair loss is stable. Don't make any irreversible decisions while you're feeling this overwhelmed.
