17 Month Update On Finasteride And Ready To Throw In The Towel

I’m now 17 months on finasteride and my hair is in the worst shape it’s ever been. I’m sure this drug works wonders for most but for me it has - at best - failed miserably and - at worst - accelerated my hair loss. I also have had alopecia areata for 25 years but those flare ups have always occurred on the back of my scalp. In my entire time on finasteride I was never aware of any unusual shedding but I was also doing my best to not pay so much attention as I understood that time was needed to see results.

When my hair loss started in May of 2018 it started at the left temple. Then the right temple. The receding occurred rapidly in the 2 months prior to starting treatment (I’d estimate a half an inch to an inch at each temple. I had no signs of pattern hair loss anywhere else on my scalp or previous to this. 4 months in I noticed after showering that the areas affected by thinning were no longer exclusive to hairline or temples as it had crept to my forelock, mid scalp and vertex.

By 6 months it was clear that almost all of the hair in the Norwood region was being miniaturized - meaning all my hair was suddenly tapered at the root and was therefore thinner in density and appearance. I honestly can’t explain how this could’ve happened although i have seen theories floated in this forum. but I refused to panic and pressed on trying to convince myself that this was a good sign and that the hair was cycling.

Well here I am almost 12 months later and my hairline and temples have receded further, my forelock now has a sizable thinning spot and my once dense hair is now diffuse thinning in the horse shoe pattern. I’d say I’m probably a Norwood 3 approaching a Norwood 4 in the coming months after starting at about Norwood 1 prior to April of 2018. So i estimate i have lost about 2 NWs in less than 20 months and on finasteride for all but 3 months of that time . There’s no way that is normal.

It’s interesting to note also that although my crown remains thinner after the initial 6 month loss it still appears to be in the same shape as it was 10-12 months ago. So to summarize my loss started at the temples and my frontal thinning has continued to get the worst of it. The mid scalp and vertex took a hit at the 4-6 month mark but haven’t changed drastically since.

As a side note I have an identical twin brother who I’d say is a Norwood 1.5 at worst with no diffuse thinning that I can see (he keeps his hair much shorter than I do). So as an added bonus to this disaster I have to live with a replica of myself walking around with great hair as I rapidly bald.

in any case I have dutasteride on its way so I feel like I have 2 options

1) taper off all AAs and try to reset my hormonal balance in the hope that the finasteride has caused me to go turbo. The fact that my brother isn’t having this issue makes me doubt how much of this is genetics and maybe AAs were not the correct path (even though 2 different derms prescribed them and my blood work was normal.

2) start taking duta and pray that whatever is causing such rapid loss is only DHT related.

The comical part of this is that I was only ever hoping for maintenance and to stop what I saw as a sudden and rapidly receding hairline. I’d give anything for my hair of 6 months ago which is what I said 6 months ago and 6 months prior to that. And I’m sure I’ll feel the same 6 months from now. This has completely consumed my life for the better part of two years and my wife thinks that i should just accept going bald. But i refuse to stop fighting this. Any opinions are welcome.

I have some other shots of the temples that I can post but I think it’s clear I’ve lost ground. Not sure If it shows but even the texture and color of my hair has changed. Some of that may be aging but that seems like a pretty drastic change in 20 months. I look like I’ve aged 10 years.

Your hair has receded a bit, but it's NOWHERE near a Norwood 4. Your hair isn't that bad honestly.

Maybe it's time for dutasteride.

Thanks for the feedback. It’s good to hear someone else’s opinion because I’ve been pretty much fighting this on my own. I will probably give dutasteride a try. Would you agree that I haven’t responded to finasteride?

Here’s a recent shot of my left temple. Receded a lot in one year on finasteride. The forelock area behind the hairline is very weak as well. Hopefully duta can help hold the line.

I do agree that ur hairloss has progressed but not as much as u are saying. Maybe add a topic anti androgen or try dutasteride.

It’s the short amount of time it’s progressed that has me freaked out. And there is weak thinning hair beyond all the hair that’s already been completely lost. I think I’ll definitely give Dutasteride a shot when it arrives (should be any day).
What kind of topical do you suggest?

I would suggest ru or cb but i personally wouldn't use either at this moment, ru because it's not proven safe and cb because it comes from random labs in China. I would wait till cb comes out officially and use it, but its probably another 2-3 years of waiting.

Yes, I would agree. However, it can take up to 2 years for finasteride to stabilize hairloss.

hey so can relate to that speed and a lack of response to Finasteride. It is crazy that there is no genetic way to assess whether or not the drug is effective.

I’m considering a topical AA maybe CB if there a decent source though I have a batch of RU.

If I could go back in time I would have done nothing. They tell you them treatments need to be started ASAP but the treatments seem to ramp up the process for some people.

I’m sure you’ve thought of this, but your hair could get much worse if you stop finasteride without switching to dutasteride. It could be the only thing holding the line against a hairline that is far worse. That said switching to dutasteride will probably give you a big *** shed, as it’s known to do, and could make things look worse in the short term. If it turns out it isn’t purely a DHT issue then it could just make things worse if regrowth doesn’t occur after the initial shed.

That being said, if it was me, I would either stay on finasteride another 6 months or switch over to dutasteride. I would not taper off in your situation. It’s clear you’re not satisfied with the results of finasteride, but it’s not definitive enough to know that it isn’t a problem an AA can fix. At this point, there’s really only one more step you can take from an AA perspective and you may as well give it a shot and find out for sure if your problem can be resolved via this path. If you stop now, you’ll never know and, if it is in fact the source of the issue and you find that out by stopping your AA regimen, you will regret it.

Thanks for posting your advice. I actually switched from finasteride to dutasteride yesterday. When should I expect to start noticing a shed? When you say it might not be a DHT issue what does that mean exactly?

So I’ve now been on dutasteride for 2 months and my situation is exponentially worse. Immediate hair loss on back of head and sides within a week of starting (pics on another thread). And within the last couple of weeks the rest of it has been decimated as well. I thought it was maintaining but after getting a haircut I was sorely mistaken. Any density I had left has completely vanished, my left temple is reaching the mid scalp and my hairline is just gone.

To top things off yesterday I noticed that my left testicle was really swollen which my dr confirmed and had me get an ultrasound which I pray shows a cyst and not cancer. Things honestly couldn’t be worse.

In hindsight I should’ve just stayed on finasteride which, although the results were very disappointing over 17 months, at least it wasn’t destroying every last hair follicle on my head and i wasn’t being sent for imaging tests for cancer.

There’s no way I’m going to come back from this. Just like finasteride initially accelerated my hair loss which never recovered the same thing is happening with dutasteride. I wasn’t happy with the way my hair looked 2 months ago but considering my current state I would do anything to get back to that point. Anything. I’m sure I’ll get comments that I need to “stay the course” with dutadterude and that this is “normal” but I’ve been hearing the same sh*t for almost 2 years now. I’ve permanently fucked myself up with these drugs. RH. Upregulation. What ever you want to call it. I’d love to get some advice on how to get off this one way train to Norwood hell but I don’t see many options other than to go back to finasteride or just quit this nonsense and let nature take course. I should’ve never messed with my hormones but vanity got the best of me and I’m paying the ultimate price for that.

You are guessing by saying finasteride n dutasteride caused your baldness. And you're guessing against science.

What's more likely is that your were losing hair quickly n went n got on finasteride because you were worried. And you are just not a responder. There is about 12% who just don't respond. To blame it on finasteride n dutasteride is just so highly unlikely you might as well not even mention it. And I've seen guys in their mid to late 40s that had decent hair, only to lose it in a matter of a few years. These things happen. Sounds like its what happened to you. Tough break.

Literally from the day i started treating my hairloss, everything went to sh*t. The more i used against my hairloss, the worse it got. And unlike you i waited not only 2 months, but many many months to see if things are working. In the case of dutasteride it was 10 months. Hairline and hair density got decimated and i waited 10 months. I didnt expect to regain everything, but when i saw ZERO signs of regrowth or recovery in those 10 months, i called it quits afterwards. Okay, maybe 10 months wasnt enough. Maybe i needed to wait 2 years. Literally whats the point of this medication (for me), if i lose 40% of my hair density and need to wait an eternity for things to get even remotely better.

I respect your opinion but Im just sharing what happened to me. I understand what the science says and I believe these medications work for the vast majority. I also believe that everyone reacts slightly differently but in some cases there is an adverse reaction. You’re right that I’m guessing at the reason for this. RH or upregulation. An immune response to the hormonal change. Whatever it is. I really don’t know or care. All I know is that I had some temple recession and 4 months into finasteride my mid scalp, forelock and crown all lost an incredible amount of density which never recovered but also never got much worse in 17 months.

If you want to say that was inevitable, a case of sudden highly aggressive hair loss which started and stopped abruptly that’s fine and I’m not going to argue your opinion. But a week after starting dutasteride after being on finasteride for 17 months my donor zone was shredded. And the mid scalp and crown which had maintained after the initial hit has also thinned out. You can go look at my pictures. There’s zero chance this is coincidental in my opinion. And I’m far from the only person reporting this type of reaction. At the end of the day it doesn’t matter what actually happened. It’s not going to get my hair back.

It wasn’t just 2 months. That was just the dutasteride. I was 17 months on finasteride. So 19 months total with exceedingly awful results.
