Yellow Bumps On The Back Of My Scalp; Random Patches Of Hair Loss There Too

For roughly three years now I've had an issue with yellow bumps on the back of my scalp. My donor zone has been heavily hit as a result of permanent hair loss in spots where the bumps are. I can't let my hair grow out though as the only way to hide my diffuse hair loss is to keep it at the lowest length possible. I tried nizoral 2% for one month in addition to topical clindamycin but neither seemed to have any effect. The yellow material is deep and cannot be extracted with any type of tool or finger compression. I cut my hair every 10 days with the lowest setting possible using electric clippers. I'm worried that if the issue gets any worse than the hair loss on the back of my head will start to draw unwanted attention. Any clues as to what the issue is? Treatment options?
