The first intimations of hair loss began for me about nine years ago, when I was 24 years old. The photograph on the left was taken seven years ago. The one on the right is from this past summer. Is it a hopeless case, or should I start saving for a transplant? Seven years is a long time to live in misery. Another five or six decades will be longer, and the situation is not going to get any better.

I would recommend you consult a doctor about trying finasteride + minoxidil first. Then try dutasteride in replacement to finasteride. If you respond well and are satisfied with a treatment, you may not need a transplant. However, be cautious of the potential side effects of these drugs. If you do not respond well, I would consult with a hair transplant surgeon to gauge your expectations. I would say though that going bald/ balding at 35+ is not the slightest bit uncommon.

Unfortunately my hair did not respond to finasteride. Back in 2015-16 i took finasteride daily for a year and the hair loss continued, so my decision was to let the illness run its course without my interference, and then consider a transplant. my father and his father never suffered any hair loss, so the gene most likely came to me through my mother, whose male sibling was a diffused NW4 or so. yet if i end up there and not at NW7, the diffusion would still make my hair look ridiculous even if a transplant were to partially restore the front and the crown area. it is pointless.

The crown and hairline would have to be restored in order for my face to look somewhat normal again, and this would cost something like 4,500 grafts in my estimate. It is probably a waste of money though because nothing can be done about the area in between which is destroyed by the diffuse thinning, so it would still look ridiculous.
