Transplanted hair falling out any idea why?

Hi I did a hair transplant may of 2022. The results were good after a full year. However it appears the hairs are falling out and I am losing density. I do not use finasteride or min. But I was told that transplanted hair will not be affected by the dht.

The transplanted hair came from the back of my head where it is very dense. Why is it that the location of where the follicle is placed determines if it will shrink and die? I thought either the follicle was resistant or not.

Because the condition on the scalp haven’t changed.. which means the area of your scalp which were previously in the Telogen growth phase are still being i signaled to not grow. Your hairs are in follicles which are controlled by hair stem cells. Basically the stem cells are in the ON anagen growth phase. DHT signals the stem cells to OFF— Telogen. So the trick is to turn ON the hair stem cells or they will deteriorate no matter how strong /vibrant they are

You had the transplant done before your shedding was over. The situation here should have been identified well.

My hair loss started in my early 20s I did my transplant about 20 years later. I don't know what you're talking about shedding. I had lost everything I was going to lose

The timing is the key here. If this is happening within the first few weeks after surgery, it is usually the transplanted hair shafts shedding while the follicles remain in place. That is a normal part of the post-transplant cycle. If the hair had already grown in and the shedding started many months or years later, though, I would not automatically label it as normal shock loss.

The first thing to establish is whether you are actually losing transplanted hairs or the native hairs growing between the grafts. Once both have grown together, it can be very difficult to tell just by looking at the hairs that fall out. A trichoscopic examination of both the recipient and donor areas would be useful to check for miniaturization.

It would also be worth reviewing any recent illness, high fever, major stress, rapid weight loss, nutritional deficiency, or change in finasteride or minoxidil use, as these can sometimes trigger a temporary shed. Less commonly, hairs taken from the margins of an unstable donor zone may also thin over time.

I would contact the clinic that performed the procedure or see a dermatologist before changing any medication. Clear comparison photos taken under the same lighting, together with a proper scalp examination, will provide much more information than counting the individual hairs that fall out.
