Abstract
INTRODUCTION:
There has been considerable interest in the interplay between testosterone and cognition. Dihydrotestosterone (DHT), which has been correlated with cognitive function, is significantly reduced with the use of 5 alpha reductase inhibitors (5ARI) for prostatic enlargement. Our objective was to assess whether the use of 5ARIs was associated with an increased risk of incident dementia.

METHODS:
We used a matched cohort design and linked administrative data from the province of Ontario, Canada. A total of 99 covariates were measured, and a propensity score was used for matching; 81,162 men who used a 5ARIs were matched to an equal number of men who did not.

RESULTS:
New initiation of 5ARI medication was associated with an increased risk of dementia during the first (HR 2.18, 95% CI 2.01-2.35) and second (HR 1.52, 95% CI 1.39-1.67) year, however this risk was nonsignificant among the men with the longest exposure to 5ARIs (HR 1.06, 95% CI 0.98-1.14). There was no difference in the results between types of 5ARIs.

CONCLUSION:
As the strength of the association decreased with increased exposure, the higher risk seen in the initial two years likely represents the presentation and treatment of urinary symptoms which coexist with mild cognitive impairment and eventually progresses to a diagnosis of dementia.

Just out of curiosity what is the longest amount of time men have been on finasteride for prostate issues or propecia for male pattern baldness? How long has finasteride existed for treating enlarged prostates are there men that started back then and still take it today?
Eh I dont know if it changes dementia, but the inhibition of 5 ar 3 and those neurosteroids is a little something I would be concerned about long term what that could do.
If they reformulated finasteride to only change 5-ar 2 I wonder if neurosteroid levels would stay the same and what would happen to the side effects

If you live long enough, you will probably get some cognitive decline or dementia anyways. Why not save our hair before then?

5 alpha reductase inhibitors may not be all bad news. Here are two studies which provide evidence that Dutasteride has neuro-protective properties in Parkinson's disease:

Study 1:

1. Dutasteride prevented the MPTP-induced striatal loss of dopamine contents.

4. Dutasteride affected androgen and progesterone metabolism in brain and plasma.
5. Dutasteride prevented MPTP-induced increase of GFAP.

Highlights:

• Finasteride did not protect striatal dopamine concentrations, DAT or VMAT2 levels.

And the last paragraph reads:
In summary, our results showed that a 5α-reductase inhibitor, Dutasteride has neuroprotective activity preventing in male mice the MPTP-induced loss of several dopaminergic markers.

Of all things negatively associated with finasteride, this is by far the least to worry about.

If your dick works, your titties don’t hurt, and you aren’t thinking about killing yourself, you’re good to go

Yes finasteride is a joke. What about minoxidil? I Don't know what to think anymore about it.
