Telogen Effluvium or Androgenetic Alopecia?

Hi all :)
I just joined and is hoping to find someone who can give me some input. Well here's my story:
I've had eating disorder for more than 10yrs. Began as anorexic when i was 13 yrs old (i'm 27 now) and went on for almost 2 yrs (stopped menstruating because of lack of nutrition) then i got help and gained back to healthy weight. Then somewhere on 2000 i had an extreme crash diet and did some crystal meth. I lost lots of weight in short amount of time. Then one day I went to bleach my hair and the next day my hair suddenly dropped. I lost about 50% of my hair in a period of 2weeks. Then the shedding gradually stopped. But my hair never fully recovered to its original density (i had stopped using the meth). It was 2000 and during that time until now i usually had an off/on period of shedding and my eating regime has never been normal. I either ate one big meal (binge) at night and starved myself during the day or i would binge all day and did some vigorous exercise at the end of the day trying to burn off all the calories from the binge. There were some period when i'd eat 3 normal meals throughout the day, but it never lasted more than couple of weeks and I'd be back to my unhealthy binging pattern. So my hair continue getting thinner during these last 10 yrs until last year i started using crystal meth again. This time the addiction was worse than before and it went on for almost a year. Meth supressed my appetite and i lost around 10kg in short amount of time. Then i noticed my hair texture is getting thinner (miniaturized) and the thinning gets worse. It's been thinning for the last 10yrs but the texture was fine before. I was depressed so i stopped using the meth and tried to eat and sleep normally (I was insomniac for years). Ate lots of protein, stopped the night binging and eat 3 proper meals 3x a day. It's been almost 4 months since i live healthily but my hair still feel so thin. There are some baby hair at the crown of my hair though.
BTW, the thinning is all over the scalp. The hairline looks worse because just like everybody we have less hair on that part of the scalp. Link to Picture below

My questions are:
1. Am i having Chronic Telogen effluvium or Androgenetic Alopecia or combination of both?
2. Can a prolonged period of binge eating causes Chronic Telogen effluvium?
3. Is there a chance i can have at least 3/4 of my original density of my hair back if i continue to eat normally (stopped the night binging)?
4. I'm going to sydney early jan 2011, could anyone recommend a good derm/doctor specializing in Hair loss?
