I have diffuse thinning all over the top and it's all red and sore too, if I move the hair it hurts. I already use Nizoral so why could this be happening?

Well, read the nizoral list of ingredients and you'll answer the question.
It contains Sodium lauryl Sulphate, which is an industrial detergent, and
even Chlorhydric Acid(!!), along with other 10+ harsh chemicals.
I don't see the point putting tons of sulfates and acids on your hair only because of the ingredient ketoconazole, which is only 2% of the composition!!
Which ingredient MAY be god for hair, no one ever proved.(like minoxidil and propecia).
I wouldn't advise using Nizoral.
Ketoconazole, alog with other good for hair substances is also found in REVITA.
Even REVITA is not completely free of shitty substances, but at least it does not contain SLS and HCL, for god's sake!
be careful with Nizoral, its main goal is to kill the fungus which cause dandruff, dermatites, sebborhea, etc, but ,along with these, it may also kill your hair follicles! And as side effects it may produce inflamation, redness, even more dandruff, etc.

Better read ingredients of all shampoo. Sodium Lauryl Sulfate is main ingredient for most soaps.
