No your Doctor doesn't need to know about it. You can speek with him about Sides if you mention.
No 1mg is the common pill. But if you want to lower the dose you can just buy a pill cutter.
On the other hand I would highlty recommend to buy the 5mg variant because its much cheaper! You can cut the pills, I cut them in to 4 quarters (1,25mg)
