I think its been said men on propecia have substantially higher numbers of receptors compared to men not on it. I know some have said its not a big deal, but the guys that partnered with cosmo for CB specifically say that balding men have over expression of receptors compared to non balding men. So it begs the question how important is a big increases in receptors in terms of hair loss? It must be important to some extent if they are pointing that out and basing a treatment on interfering with the androgen/receptor interactions.
Your thoughts? How big a role if any do the number of receptors play in hair loss or are there any good studies done on this or how propecia changes this?

The androgen receptor is part of the set of factors that are necessary causes of male pattern baldness. That is without the interaction with the androgen receptor with 5-alpha dihidrotestosteronethere would be no androgenetic alopecia.

What is interesting is that androgen receptor becomes upregulated with usage of 5ar inhibition therapy, which I suppose explains the gradual decline in efficacy that some report with finasteride, etc.

Are there any studies on exactly why propecia causes upregulation of receptors or why some people might have more upregulation then others? Does the upregulation happen everywhere equally all over your head or is it more in certain spots? It seems like Merk would have at least looked at this considering it seems kind of important

Upregulation is the reason why some people choose to take finasteride in on/off cycles, like 3 weeks on, 1 week off.

I find this interesting. So it is not all about inhibiting DHT, we knew that. Working with the AR is important too. Is that where spironolactone comes in?
