I would start Minoxidil foam on the top and Nizoral shampoo and caffeine shampoo and monitor the hair line. If the hair line is continuing to decay then consider Finasteride.

Diffuse thinning. Get on finasteride asap. That's all you can do with diffuse thinning. That, or slowly watch your hair thin out everywhere and go bald painfully and (sometimes) slowly. I had a friend with a low forehead and a NW1 hairline, but he has been experiencing diffuse thinning,worsening every year, since his teens. It's finasteride or bald, my friend. I am deeply sorry.

Did you not notice the thinning on his scalp? He's a diffuse thinner at the very least, albeit maybe in the early stages. Min might buy him a year or two. But if he doesn't go on finasteride, he's going to go Norwood 7.

Why? Because you mom had sex with your dad and the combination of their genetics they gave you makes your scalp more sensitive to DHT than which is normal for your age.

I was thinning when I was 19 on the top and used Minoxidil alone for six months and was fine for ten years. I am sure you would have told me I needed Finasteride as well.

Your case is weird though dude, you have to admit

My point is not everyone who thins is destined to be a NW7. This guy has thinning but his hair is wet so makes it look much worse. I am not saying to not do anything, but hold off on the Finasteride until he KNOWS he needs it.

I have had a NW1.5 for at least a year, maybe longer. When I first noticed it, the doctor wanted me to go on Rogaine immediately. I was hesitant, and just kept some on hand in case I started to see any thinning. A year later, nothing. In any case, I can tell you that when I get out of the shower, I have a fat mop of wet hair with no scalp showing anywhere. As far as I can tell, I am not actively receding. If you have thinning that far behind your hairline, and with that much thinning in a large area, I think it's self evident that balding is inevitable. Maybe not NW7, but could end up being NW4a. No mature hairline (which I am fairly sure that I have) has thinning like that, especially that far behind the hairline. If his thinning was more widespread, I would either assume full on diffuse thinning or naturally thinner hair. Nonetheless, I think it is quite obvious some significant balding of the top is inevitable in the future. He should justget on finasteride now to avoid regret later.

Would you have preferred to get on finasteride then, though, in retrospect? I am trying to save this guy some psychological trauma down the road.

No I am 30yrs old and just started Finasteride. Why would I have preferred to take Finasteride for 10yrs?

Fair point, I suppose. finasteride's certainly not child's play. Still, this guy's hair loss to me is very indicative he will eventually go bald at the very least on top.
