Anyone know what happened to the form here on HairLossTalk.com to fill out for the new Merck study with the super duper 1000 x minoxidil stuff? The page source has the form but it doesn't appear...

That was bristol meyers and it turned out to be a rather unsafe chemical that apparently didn't do well in trials about 2 yrs ago.
