New Japanese Propecia study.

Finasteride is a type 2 5 alpha-reductase inhibitor that inhibits conversion of testosterone to dihydrotestosterone, a key mediator of male pattern hair loss (androgenetic alopecia). The objective of this study was to identify the optimal dosage of finasteride and to evaluate its efficacy and safety in the treatment of Japanese men with male pattern hair loss. In this double- blind randomized study, 414 Japanese men with male pattern hair loss received finasteride 1 mg (n = 139), finasteride 0.2 mg (n = 137), or placebo (n = 38) once daily for 48 weeks. Efficacy was evaluated by global photographic assessment, patient self-assessment, and investigator assessment. All efficacy endpoints showed significant improvement with finasteride therapy by 12 weeks (p < 0.05 versus placebo). At 48 weeks, 58%, 54%, and 6% of men in the finasteride 1 mg, finasteride 0.2 mg, and placebo groups, respectively, had improved based on assessments of global photographs. All efficacy endpoints were numerically superior for the 1 mg dose over the 0.2 mg dose at 48 weeks. Finasteride treatment was generally well tolerated. Finasteride 1 mg\day slows hair loss and improves hair growth in Japanese men with male pattern hair loss.

well, at least in japan, people know how to make use of their finding and data. therefore they come out with the 0.2mg version of propecia. unlike some folks who insist on 1mg or nothing else will work mindset.

People take the 1mg position because it works better.

Technically nobody has said lower doses don't work. They've said lower doses work significantly less than 1mg. And its your choice if you want to risk your hair.

Its not a fight between the "1mg-ers" and the "other doses-ers". Its simply that the data shows 1mg works best.

I was on 1mg for 2.5 yrs and lost my results when going down to 0.5mg, so ... yeah.

i have never doubt that 1mg work better than 0.2mg. but the question is by how much? if we can do with half the dosage yet get the same result, why not? by increasing the dosage by 5times the different between people who grow hair is only 4% in the study? of course we still have to take into consideration on how much hair grow different which was not mention.

here Mercks response to this issue: -in agreement with the superior efficacy of the 1-mg dose, serum DHT was suppressed more with the 1-mg dose than with the 0.2-mg dose (medianSE percent decrease, -68.5%1.4% vs -61.2%1.7%....Frankel goes further and recommends that men with male pattern hair loss take the lower (0.2-mg) dose of finasteride, a dose that is suboptimal and is neither approved nor available to patients.-

not trying to get into a debate here. just that with the possibility that slightly lower dosage can serve the same purpose, maybe it worth a try. but to take note the risk is there as HairLossTalk.com have said.

anyway HairLossTalk.com, could it be that you are already on 1mg for 2.5years and your body is producing extra 5-alpha testosterone so when you reduce it to 0.5mg, you experience increase in hair loss?

The 7.4% increase in serum DHT doesn't necessarily translate into only a 7.4% reduction in effectiveness via haircount or stabilization. The reduction in effectiveness could be much higher. We all differ in response, and it could be that many who have success with the 1mg dose would lose results at .2mg. HairLossTalk.com sounds like he's one of them. There are likely a significant proportion of others out there.
