I’ve been reading this forum for a couple months now, but I feel like now is the time I need some advice from you guys myself. This post is gonna be a bit long because I wanna cover my full situation. I hope you bear with me anyway! Also sorry if my English isn’t all correct, I’m not from an English speaking country.


So I’ve been diffuse thinning rapidly for basically one year now. It started last year November and a few months later I was already making plans when I was gonna shave my head because the hair fall is insane. Please don’t think I’m exaggerating, but it’s literally hundreds a day. I can easily lose 200 shampooing/drying/combing my hair and then when I apply hair product after I can still lose 20/30 easily.. I don’t know how, but I’m honestly surprised I still have a single hair on my head left seeing it all fall like this. I never counted a whole day, but that can be in the hundreds. Also I have a hairstyle where I leave the top long and sides and back kinda shaved, so most of the hair I see falling out must come from that long area as I hardly see little hairs on my hands or in the sink...


I’ve had the basic blood work done for thyroid, iron and stuff like that but nothing came out. I’ve seen a dermatologist, but he doesn’t see the problem. He says my hair looks okay now so I should just give it a while. He’s willing to prescribe me meds, but he’d rather want me to accept that I’m balding.


Right now, I think my hair still looks okay. It’s thin, but it has always been thin I think. My hairline is sky high, but it’s always been like that as well. When I lift my hair up, I can see straight through it and I feel like you can really see it getting thinner on my crown as well since it just covers my crown differently and not as good as it used to.. On the other hand my hairdresser and dermatologist acknowledge my hair is a bit thin, but say it looks good and it’s not like I’m balding. Although I really think and see it getting thinner, my hair (surprisingly) still covers it well now and you wouldn’t suspect I’m balding yet. This makes it hard to keep an eye on things, diffuse thinning is just hard to see until it’s too late I guess..


I’ve wanted to start finasteride for a couple months now but I just don’t have the balls to do it. For some reason I feel really bad about altering my hormones, feel like I’m giving up some kind of my manliness and I’m terrified for the sexual sides. On the other hand, living the bald dream is scaring me even more and I just don’t know what to do now. All I know is if I do nothing at all, that bald head is going to be reality soon.


I also have mild seborrheic dermatitis according to my derm. Used the 2% keto shampoo for it what worked fine for that but did nothing for my hair situation. Also when some hairs fall out, I sometimes see some little white bit covering the bulb. Is that the seb derm from my head covering the bulb or?


And the hairs that fall out vary in form. Most are just long normal hairs but some are different. Some have a weird twist to them, thinner at the end and some are just thin and small in general. Also some weird hairs with a corkscrew like shape. Is that the DHT that affected the hair follicle? Does every full head have hairs like that as well or does that only happen to people with male pattern baldness?


Is there anyone that recognizes this and is going through the same? What do you guys think I should do right now?


Also I didn’t add pictures because it’s hard to capture this diffuse thinning in a picture. Can make them if really needed! Temples are still decent I guess, don’t see much difference. I’m 21 y/o right now, have a bald dad who started mid/late twenties and both 80 y/o grandpas are kind of bald as well.

Either get on finasteride or go bald?
If finasteride scares you so much you don't want to take it then the only other options are a wig or smp.

Sure get on finasteride for male pattern baldness, however seborraic dermititis can make you lose alot of hair as well. and nizoral may not be enough{(it wasnt for me) . id add coal tar and possible selsun gold, and alternate with a non chemical shampoo. you can also do salt rinses which are very good for it. once the seb derm is under control, then maybe finasteride will work. the idea is since seb derm is a fungal infection, you have to keep swapping chemicals so the fungi doesnt get tolerant.

It is definitely male pattern baldness. We even have a similar family history of hair loss (my dad was nw7 in mid 20s) and we are the same age. If it is as aggressive as you are saying then get on treatments now but keep in mind that even with these medication you will probably still continue to go bald.

@recedingornot87 Oh man, that is brutal, I really feel you.. How’s the meds working for you so far and are you already 100% sure it’s male pattern baldness and not some deficiency or Telogen Effluvium?

And yes, it’s pretty aggressive I think. Like you and others commenting on your topic, I see hairs everywhere and after taking a shower it’s like someone’s been combing his cat in there. Still I have decent hair on top and I think that’s what holding me off meds. I’m scared shitless for fina but I’m pretty sure if I can see more scalp, i would be popping those pills straight away. Also my derm keeps saying it’s okay and I need to stop worrying but that’s really contradicting to what I see happening..

Did you see miniaturized hairs between the falling hairs as well?

@anxious1 You think seb derm could cause this type of hair fall? It’s pretty mild I guess, it’s not like I’m shedding flakes everywhere, it’s just that there’s some stuck to my head that comes off with my finger or comb.

From the sounds of it you have a more aggressive form of male pattern baldness. I mean this shouldn't be a surprise considering your dad and grandad right. You will have decent hair up there for a few months (maybe even a few years sometimes) but then it will go to sh*t, trust me ive been there. I was able to shed like that for months and months before I noticed a cosmetic difference. It will seem almost overnight when you wake up and start to see scalp in almost every lighting but it happens over time.

Unfortunately, your derm probably doesn't know as much about male pattern baldness as you think. If you go to somewhere like a hair transplant clinic they will be better at confirming if its male pattern baldness by using something called a haircheck instrument and they will be much more knowledgeable about it.

In terms of miniaturized hairs, I didn't see them at first when the loss started but I am seeing them more and more now.

Hmm difficult! Yeah I knew this was somewhere down the line for me as well, but I was hoping to enjoy my hair for a couple more years.. I think it’s pretty aggressive based on the thinning and hair fall, but apparently my hairdresser and derm think different...

I might check out some hair transplantation specialist in my area, see if they’re willing to check my scalp!

I would definitely find out for certain if it is hair loss for sure. From there, if it is, I would strongly consider 1mg finasteride. Also worth adding nizoral shampoo 1 or 2 times a week for your hair condidtion.
