I like your plan of restarting Nizoral, minoxidil, and dutasteride (or finasteride) at the same time. I think that will give you a chance to test and see if they have an effect after 6 months to a year.

This is just a theory, but I would think it is possible for a treatment to be doing something even if you don't notice it. Think of it as a BATTLE between the hair loss process and the treatments. The hair loss process is "fighting" to weaken your hair, while the treatments are "fighting" against the hair loss process. If the treatments are stronger than your particular hair loss, then you'll notice net hair growth. If the treatments are weaker, even though they might be doing their job, the hair loss process will still be "winning" overall. However, it will be "winning" in a much slower fashion. If you used NO treatments at all, the hair loss process would be much faster. The idea is to add treatments until the regimen matches the hair loss's strength and brings it to a halt, if possible.

I like your overall plan, except that again, I don't think it is necessary to take BOTH dutasteride and finasteride at the same time. That seems to me to be unnecessary, and possibly dangerous. Why not just use finasteride for a year and see how that works? If you're not happy at the end of the year, then switch to dutasteride for a year. Then you can compare both at the end of the two years.

You could potentially try a higher concentration of minoxidil, though I am not sure how safe that is. I think 5% is known to be effective and relatively safe.

I agree that dutasteride shouldn't thin the frontal section of the hair, but remember, if you haven't used it for a period of at least 6 months, you cannot make a proper judgment about its effectiveness. You should EXPECT some initial thinning and shedding, as the dormant telogen hairs are getting pushed out to make room for new hair growth.
