I’m honestly not sure what to do. I’d love to hear people’s opinions/advice here. I’m not even sure if a trichologist could help, as reflex hyperandrogeneticy isn’t well studied.
If you hair is worse you should quit before its too late. I wish I quit after a year. Not continue for another 5 months. After I quit my hair thickened up slightly. So you see..... I believe DHT isnt the main cause of my hair thinning at all..im 38 soon and started losing at 22-23. Still got alot of hair.

I also have thinning beard now after dutasteride, so idk if it's reflex hyperandrogenecity or what

The reason I switched to Dutasteride was because Finasteride wasn’t stabilising my hair.
I see. Well if things are getting worse on dutasteride, there are only 2 possibilities. 1, you are balding so aggressively not even a strong AA can stop it. 2, you do not respond well to AAs and they are further accelerating your loss.

I’m even taking RU 5%, but not even that is halting the thinning. I do notice new baby hairs from 5mg OM and RU, but without stabilising, there’s little point. Maybe my hair loss isn’t so much DHT related, perhaps there’s something else I’m not addressing? Thoughts? I don’t know what the f*ck to do anymore. I honestly feel like crying. I’m ugly without hair.

You are correct. The only thing that truely worked to stabilize my hair thinning was to nuke these damn demodex mites.

Switch back to finasteride.

I’m thinking scalp T plays a large roll considering it’s still acting on the androgen receptor, just at a less potent effect to DHT. Even RU isn’t halting it. I’m just cursed dude. I fall into the 10% of men who don’t respond to 5ar inhibitors.

Same here dude. I was on dutasteride for 1.5 years. All it did was reducing my semen production and gave retrograde ejac. Hair? a thin mess.

Were you taking 5mg of OM back then while on finasteride?

Many people do in fact respond worse to dutasteride. Hope it’s not accelerating your loss even more than finasteride.

Sometimes people seem very sensitive to T. In that case, dutasteride only spikes it more. Finasteride still moderately increases it. Maybe you should hop off 5ARIs all together. Hard to say for sure.

Thing is, I don’t want to “experiment and see”, because I can’t afford to lose more ground. If I were to switch back to finasteride, what is the best method to go about it?

What is your family history like?

Dutasteride is going to take several months to completely clear your system and for DHT to return to baseline, which takes about 6 months. But after 2 months and before 3 months, DHT levels usually return to the same levels as finasteride, around ~60-70% still reduced from baseline. If you are going to stop dutasteride you can do it cold turkey, and perhaps start finasteride after 1-2 months.
