I am not sure if DHT is the culprit

Some peope are inhibiting DHT almost completely through Avodart and yet are going balder by the day.

At 0.5mg/ day avodart reduces serum DHT by 90% and scalp DHT by only 54%. Serum testosterone actually increases 27% on .5mg avodart while scalp testosterone rises 100% with avodart (If you believe the numbers reported by Rittmaster of the Phase II avodart trial). So, there is still DHT in the scalp and testosterone in the scalp is increased... This level of blockade is not perfect and may not lower androgen stimulation below the threshold some succeptible men need to keep their hair once the process begins.

In any case, DHT DOES have a role in male pattern baldness, as reduction in DHT available thru blockad of the type II enzyme with finasteride does affect hair loss. Men born without this type II 5AR enzyme do not develope male pattern baldness despite having testosterone. This shows that DHT is critical for the expression of male pattern baldness.

Manipulation of androgens (dht, scalp testosterone) may not be enough to prevent male pattern baldness on its own even with avodart at higher doses, but there are other meds that have an effect on hair growth (minoxidil, copper peptides) which combined with the best available reduction of DHT may help more people grow hair.

I agree that avodart .5 mg /day is not the cure for all hair loss, but higher doses and combination treatments may have different results. In any case DHT is an important part of the puzzle...
