Just wanted to know where i can buy dr lees product? Do i have to send back that form he sent me in the email before I can buy it?

If you're in the USA you have to fill out that form and mail it to him (not email). After they receive it and get your info processed they'll send you an email letting you know you can purchase off the site.
There will be a $20 consultation fee applied to your first order.
