how come we don't lose hair on sides and back

as far as i've ever heard it, the theory is just that those areas are less sensitive to DHT. some people however do thin out on the sides and back.

is there a difference between the hair on top of the scalp and the hair on the sides and back?

I don't think there's ever been a definitive proven answer as to why the horseshoe area is resistant to DHT or male pattern baldness. It's mysterious, especially as the outline so precisely defined.

some have noted that the Norwood area coincides with the Galea, and have postulated that that may have something to do with it
