How bad is my diffuse hairloss getting?

Nobody can tell I am thinning unless I pull my hair back, but I have noticed all over my head (the nape of my neck, the sides, the top...etc) my hair is feeling thinner and thinner. I have genetically fine hair, but now it kind of feels like nothing is there :O. My sideburns are just one or 2 hair strands these days. I have attatched some pictures to show how bad it is getting. Behind my ears is partically bad I think.

Another thing I should mention is that hair over my entire body begins to itch after hot showers or sudden changes in temperature. All over my scalp is itchy, including body hair and facial hair. Also when I itch I feel little white dots, I guess like dandruff on my scalp, but it's like stuck into my scalp. I can pick them off my scalp, but I'm not sure if I should be doing that. It might have something to do with the hair follicule, I'm not sure. Also, my scalp feels tender and sore at times, I'm not sure what's causing this. If any of you have any insights into this, please let me know. I have posted previous pictures of my hairline on here before, and I don't think it's too bad. I reckon between a NW1-NW2. I've seen people who have worser hairlines than mine, but generally thicker hair where they do have hair.

I have been using alpecin caffeine shampoo, but I have seen very little improvement. Also tried nizoral, and it didn't even stop my hair from being itchy (used for a couple of weeks), so I'm not sure what to do. Maybe I am screwed. I don't want to try anything else, because I heard they have can have sexual sideaffects.

P.S. I had 3 large sebaceous cysts surgically removed around 1 year ago. After taking the stiches out, I never went back to check for infection (apparently I was meant to). I'm not sure if this could have caused the thinning. I doubt it. I looked back and posted in the last thread you made. Anyways, have you been to your doctor/dermatologist or a hair surgeon yet?
You could be possibly suffering from something other then male pattern baldness.

Retrograde Alopecia
Diffuse Alopeca Areata (icognita)
hyperthyroidism

I'm probably missing a couple.

My advice go see your doctor or dermatologist, or a hair transplant surgeon for a better evaluation.

It seems like you have seborrhea or seborrheic dermatitis. You've tried Nizoral but it's far from being the only treatment available for this, you have to try several products to see which one works for you : shampoos or lotions containing ciclopirox olamine, piroctone olamine, zinc pyrithione, salicylic acid, coal tar, or ketoconazole (Nizoral).
Eat well, sleep well, you should also take finasteride, sides are rare and there is no scientific evidence of persisting side effects after discontinuation of the drug.

That doesn't look like typical male pattern baldness. Go see a specialist for a proper diagnosis

Can be the seborreic dermatitis. I actually had extreme amounts of sebum on my scalp, it was always inflamated and it was painful to touch. Shedding was extreme.

Finally decided to hop on shampoos and ketocanozole; started applying it religiously on the scalp and taking precautions. Also am on accutane 20 mg ED and sebum looks dry as hell but no sebum. Shedding actually stopped lol.

I think perhaps it could be seborreic dermatitis and maybe also male pattern baldness. I am definitely losing hair at my temples, so that is in the typical pattern I guess (i.e. male pattern baldness). The thinning on the sides and back would suggest something else is at play as well though.

Here is a recent picture of the back of my scalp, it is quite clearly thinning. All across my head it seems there are just a few strands. In daylight it doesn't look bad, but under a light as in these pictures, it is quite bad. It isn't just the light though, definitely losing density because it feels almost as if nothing is there when I push my hand through my hair.

Also, my hair is so thin that it is difficult to differentiate between my facial hair (which is very thin as well, will never grow thick no matter how long I let it grow) and where my scalp begins (sideburns).

I haven't been to a doctor because I cannot afford to go and see a hair dermatologist. My scalp is always itchy though, and always kind of dry/greasy. Even nizoral doesn't stop the itching. I haven't tried Coal tar, but apparently that is slightly carcinogenic, and I don't want to mess with my hormones. I'm not willing to sacrifice being a male (by removing a lot of male hormones) in order to maintain hair, so propecia and so forth I don't want to take.

I just worry that in 10 years (or less), I will be completely bald even past Norwood 7 because my back and sides are thinning :(

Oh, and one last thing, although it is difficult to see in these pictures. When I'm close up, I see a lot of copper coloured hairs and grey hairs within my scalp hair. Why is this? I've noticed grey hairs since I was 16 or so (22 now). Also the copper coloured hairs wtf are with them? I have dark brown/light black hair. My mum is ginger, but I don't think that will cause copper coloured hairs, will it? I'm not sure if you can see them in the pictures above. In this one these copper coloured hairs are clearly visible. Does this have anything to do with hairloss?

They're kind of ginger/coppery whereas the rest is dark brown/light black (I don't say black because true black hair is like asians...etc have).

Finally, I don't really use the norwood scale so much. I try to look at older pictures of my hairline, and see the difference between now. That's the best way to see if you are thinning. People have different hairlines/skull shapes, so norwood is not always the best indicator.

This is a recent picture of my hairline close up. The hair is very thin, like all over my scalp.

I still think it might be male pattern baldness because of my remple region thinning. I believe these pictures definitely show temple recession, and I put in a green line in the pictures below where I believe I should have hair. Perhaps I have a combination of seborric dermatitis and temple thinning? Not sure though, but it seems wherever I itch I lose hair, and now it seems to be everywhere on my head :(

EDIT: I just saw the comment about th copper coloured hair etc. You and I have exactly the same hairloss except i’m female. Those copper coloured hairs are hairs that are minturising. They are that colour because they are half way shrunken. I have exactly the same issue. There’s a sea of them. Also your nape and sides look like my hair. I have diffuse + retrograde pattern. Get on minoxidil now. AA’s are unlikely to help you, but I would do fina also.

What’s the significance of copper hairs? I have/had jet black hair. Recently I’m getting completely red hairs falling. A friend asked since when do you have red hair... some in beard also?

Eyelashes? I mean I never had a beard so I can’t really speak on that account, but I can 100% say that my eyelashes and eyebrows were (are) effected in the same way my scalp hair was. Do your parents have hairloss, and if so, do you notice that their facial hair is thinner? I have come to realise that my father who has Androgenetic Alopecia his this similar issue, and my mother after menopause also got thinning of facial and body hair. I think I’m just a genetic nightmare. It’s good to do some investigating so you can better understand why and how things are happening.

I get them particularly on my hairline. Like very fine copper hair. They’re miniaturising. Because they aren’t full thickness, they lose full colour.

You need to get some blood work done at your normal doctors. It could be anything really! If i was a betting man i would say your lacking vitamin D but that is just a guess.

Yup. And this is a good example of how DUPA is different to retrograde. With retrograde, it would look the same but with recession all around that hairline. I feel for you.
