That looks like acne on your forehead which is correlated with future baldness for some unlucky folks.
On top, I am not sure that you are balding but the temples don't look good to me but I haven't heard of this for people under 14 before. I would definitely see an endocrinologist soon. Dermatologists are not equipped to deal with what appears to be either early onset baldness or hopefully is just an hormonal imbalance or Telogen Effluvium.
