Fine In May, Worse In July (transplant/regimen)

Hey guys,

I´m a 23 year old male that has been balding since I was 19.
I had 2 hairtransplants over the last couple of years. One was in 2013 (1500 grafts) and one in 2015 (3500 grafts).
I remember sitting on my bed in May 2015, crying not knowing what to do anymore. Time to take action.
So we went to a clinic in Veldhoven, the netherlands, and had my 2nd hair transplant there. I also had my first one there. The clinic has very good reviews in general and people on a dutch forum were quite pleased with it!
They assured me I would be pleased with the result. I only would have to worry about my thinning at the back of my head. I still manage to hide that ''bald spot'' with toppik.

Back to the point. It has been one year since my hair transplant and things were looking really good this last april/may. I thought I would finally be free of the stress, anger and insecurity. But since the end of june/july my hair looks a lot thinner again. Especially the sides and middle. They put transplants there but I can't really say I see much of it! Altough I can't say I shedded that much hair.

I'm currently not pleased with my hair transplant. It doesn't look super full at all. It doesn't look too bad either but it's nothing something I would spend thousands of euros on.
Next month (month 14) I should make an appointment for my last check up.

What should I say to the clinic? Are there still things that can somehow improve?

Tl;dr

had 2 hair transplants (5000 grafts). Looked great in april/may 2016 . Looks worse now (june/july/august 2016)
What should I do or say at this point?

My regimen:

-5% minoxidil, Rogaine
-Saw Palmetto (640 grams), started higher dose recently
- Foligain hair capsules, started recently
-Keto shampoo 2-3 times a week

Note: I used finasteride at the start of 2013 for about a week. My girlfriend wasn't too pleased (if you know what I mean).
Noticed my behaviour change aswell, I think it was Finasteride but I'm not sure.

The first 2 pictures are from july and yesterday, last 2 were from april/june
Like I said, things were looking good last may/april. I know I won't regrow much on the back of my head but I don't care too much about that!

Bro I'm sorry to say but your July pics are not looking good at all, pretty much close to R.I.P. since ur not gonna take finasteride and already had 5000 grafts.

In my opinion any surgeon who accepts to work on a very young patient with aggressive hairloss and who is not on finasteride should be scalped.

Get on finasteride the side effects are incredibely exagerated online, and now you're cut at a young age, you must fight your hairloss! No more transplants, get a SMP on the thinning areas.

I wouldnt have guessed youd had two transplants. I think you moved too quick to get them and at your age it looks like you have agressive loss.

Hairtransplant without finasteride is like buying a chocolate kettle

So we can all agree that it doesn't look too well...
The thing is, did I lose that much hair in 2 months and what the hell happend to my transplants?
There is no change from month 8 to 12?

I'm gonna guess its your native hair progressing in the recipient area. You never mentioned of type of transplant procedure you had. If it was FUE could it be possible some of the donor hair was harvested out of the safe zone?

It was FUE, mainly focussed on the middle, sides and a bit in the front (had 1st procedure there)
But something like that shouldn't happen... They are qualified doctors and they got this quality label and stuff.
What should I tell them or what should I do?

Nothing. They should have told you to get on finasteride. You are young with aggressive male pattern baldness. finasteride is mandatory if you chose to go down the surgery path.

How about topical finasteride? Does that work and does anyone have experience with it?

Update:

Going to make an appointment tomorrow and have a check up.
See what the results are so far and maybe see if we can get some medicine/treatment going.

To be honest, I've never used it, but I intend to add it to my regimen, on top of finasteride, because I may inject some testosterone in the future.

There are many guides on how to make your RU, most people advise to order it from Kane. It's not that complicated to make according to most users, I only know one IRL. If you are that afraid of finasteride(keep in mind the placebo group had almost the same sexual side effects incident rate in the trials), it's your only solace. There is absolutely NO WAY to stop aggressive hair loss in a young subject without the use of potent anti-androgens, and since you've gone under the knife (twice) and are already losing ground you cannot afford half-measures.

I agree with you man. I don't want to wait much longer but I'm curious what my transplant doctor will say next monday. I'll keep you guys posted.
The thing is, when I was on finasteride I noticed mood swings and feeling not feeling attracted to anything anymore. A few weeks after I got off those feelings came back. I really do want to take it but reading it up on the internet and my personal experiece I'd rather try the topical variant. I hope it's nearly as effective though

I would make sure I'm on the safe side at your age after two procedures and some further loss. But you don't seem to have that mindset.

I lost so much time and precious hair because I was in denial and reluctant to take finasteride(poorly informed). I hate myself everyday for that. Don't make that mistake dear lurkers.
