Do you have to have a prescription for Nizoral 2% Shampoo?

No, you don't need a prescription.
I buy my Nizoral from my local pharmacy, but I'm sure you can get cheaper Nizoral from ebay etc...

i think it depends where you live. you dont need one in the uk they sell it in every chemists.
