Dark pores

"My Question is this: I have been experiencing hair loss for about 4 years now. It started when I was about 17 years old. And really started to get bad around the time that I began weightlifting seriously. I am now 21, and am about a northwood 3 with a rediculous amount of thinning through that middle strip that I have left in the middle. The hairloss is also very diffuse through my entire head especially in the middle and all the way back. I wear a hat 99% of the time too. I was on 80MG of Accutane for two seperate time of 6 months. once at 14 or 15, the other at 16. My scalp is very very pale and every single pore is very large and dark on my scalp. Blackheads I think they are. They are not dark where the hair has gone completely bald, only in the ares where hair remains all over my head, and they are extremely dark around the hairs that are in the middle front strip. If you put pressure like you were popping a pimples or a blackhead on my scalp anywhere like you were doing in on your face, you get the same reaction. Out from every pore or follicle can be squeeze a solid little yellowish white piece of, what I'm assuming is sebum. I don't practice this often, but the few times I have done it, a hair has come out with it. I have been taking propecia for 8 months now and minoxidil for about 4 or 5 and have seen almost no results, possible worse than what I had before. Is it possible that the Accutane has damaged my hair follicles or shrunken them? I often wonder if I have an androgen disorder because I have rediculous muscle mass and I haven't worked out in about 2 years. I also have Pubic hair think arm hairs. Very sparse, coarse and long. I have to shave them. They can be inches long if I let them. Thanks for your time, Dust hurting up North. "

hi!

well, i think you maybe right. it maybe acne on your scalp. its plausible considering the severity of your outbreaks (two terms on isotretnoin). i think you go back to your dermatologist to seek advice.

i have moderate acne on my face around the chin and underneath. i have purged thousands of pustules over the past 6/7 years and it hasnt affected my beard growth. this is good news, because it shows that your hair follices remain intact. So potentially you have lots of healthly hair, just itching to get out. having said that, it would be prudent to stay on propecia considering your high androgen levels.

do you feel that propecia has reduced the amount of androgen dependant body reactions eg . sex drive, oily skin, muscle mass??

but on the question of wether accutane has damaged your hair follices, unfortunatley i do no adequate knowledge. i have heard that accutane can cause temporary hair thinning but thats as bad as it gets. i have also heard something along the lines of prolonged use of retinoic acid (which is related to accutane) can cause androgen receptors to shut down (by 30-40%). this should mean that you shouldnt have really have male pattern baldness.

isotretnoin is very powerful.

i sincerely hope you have adequate support from your parents and family and doctor during these trying times.

I have been to the derm, but that was about 3 years ago at least. Is it true that androgen is what stimulates sebum production? Could this be why every pore has a blackhead in it?

yes, testosterone and dihydrotestosterone are largely responsible for acne, excessive sebum production, maintining muscle mass and bone density and secondary male sexual characteristics such as (sexual) hair and virility. and that dreaded male pattern baldness!

so YES! go to your derm as soon as possible. you should go to your derm at least every 3 months due to your androgen disorder.

But my blood tests always come back that my testosterone is normal!
What could they do?
Also, does masturbation frequency affect adrogen levels, like over a long period of time. If you did it excessively. I find when I go 3 or 4 days without it, I can run longer, and I'm less fatigued. Maybe I'm hyper sensitive to the physiological effects that a lot of sexual activity can have on the endocrine system?
You know when you pop a blackhead or squeeze it out? That hard stuff that comes out? mine are the size of rice grains and on some says I can run my hand through my hair and many of these will stick to my hands and greeeeeese.

okay. the majority of people that suffer from acne have androgen levels that are considered within the normal range.

the problem is that you may have receptors for those androgens which are hypersensetive to them. so in effect, you have too much androgens for your own body. whereas the same amount of androgens for someone else would be considered normal.

about the sex/masturbation question. i dont know. im not qualified to answer that right now. i suggest you propose these questions to your dermatologist.

the rice grains (accumulated dried sebum) are typical.

if i was you id try and lower my androgen levels, but this is dangerous and can have an array of adverse effects.

drugs like spirolactone, androcur (low dose) maybe effective in controling symtoms. its also worth noting that your testosterone will decrease as soon as you hit your 20s, so things should start looking better.

Got blood tests back today.
Bilirubin still high.
Testosterone was high, 120 something. Whatever that means.
He sais it was double average.
Endocrinologist appointment on the way hopefully.
Who can comment on this in relation to my original post?
