In the ER youtube clip he does have a widows peak, but, he also is very clearly receded. If he was shaving off the widows peak to match the recessed parts of his hairline, his forehead would be monstorously huge. You can see it very clearly at the 1:00 min mark. If you look at shots of his hairline now-a-days, it's very straight and very low. He definitely had some kind of work done, at least at the hairline. No way he could look like he does in the ER clip in 1999 and then 12 years later be sporting a perfectly straight hairline as low as it is.
Check his hairline in this youtube clip at the 4 min mark where he has his hair combed back. This from 2008. It's a completely different shape than in 1999.

I just think if he was wearing it would get out because every make up artist/hairdresser who's ever worked on him (that would be 100's) would know about it. And given his status as one of the worlds biggest hearthrobs someone would blab.
As for the widows peak its common for men to lose that with age just as its common for men to get a mature hairline that goes from straight across to curved at the temples.
Most wig wearers keep it long and hide the hairline but his is pretty short and the hairline is exposed, without the lace ever being spotted (like Travolta's often is).
I think he's just got awesome hair genetics. If he came out with a long feathered fringe ie like Enrique then I'd think again.

I don't think he's wearing either. Like SAF says, surely someone would come out with it or there would be more suss photos on the net or an ex girlfriend, make up artist, fellow film star would open up about his hair.
I've seen ER from the mid - late 90s and agree his hair is in better shape now than when he what looked like a NW2.5 in ER. If he shaved the windows peak, hairline area, I don't know but his hair does look thick all over regardless of a maturning/receding hairline.
If he's done anything, he's had his hairline touched up and brought forward.

I don't think he's wearing. Those videos from the late 90s show receded temples, I'm not sure if he shaved them or what. Either way, his widow's peak and the rest of his hair seems completely stable and he has great hair genetics. He might have had temples touched up with a hair transplant, who knows
