Long story short, 29 yo Indian with lifelong digestive and sleep issues, diagnosed with Hypothyroidism(Hashis) about 5 years ago. Delayed starting on hair loss meds since i thought it was more related to my thyroid. Labs came back good and still continued to lose hair. I am sure stress plays a big part too due to my lack of sleep.

Was on Morr F for a year didn't notice much regrowth but maintained, was still shedding and losing ground so thought it wasn't working and stopped (Big mistake i know). Had a lot of nutrient deficiencies at the time as well so i thought my hair hair loss was due to that+thyroid. Had mother of all sheds for last few months and I am looking to get back on meds. Blood test showed DHT at 90 at a max range of 85. My estrogen was always low at 20-25, maybe that's why the hair loss on temples?

I smoked as well, gave up and vaping now, think that might be affecting my hair loss( i know it does just not sure how much) so looking to quit that as well. Anyone else on the boat?

One thing i noticed, when on Morr-F my T was off the charts 1300 or so even Free T was outside range. Never knew Morr F goes systemic but now it makes sense. Attached some pics of current state. Still shedding a lot.

Also, I lose hair on the sides as well and it's visible, some hair loss on the back as well on the bottom area. So not sure how much hair is available for a transplant..

Started 1.25 mg proscar and no sides except balls hurting for few days, have stopped for a couple of days and it went away. Looking to start back again.

Regimen I am looking to try -
Oral 1.25 finasteride
topical - Stemoxydine (serioxyl)+RU58841 50mg.(order on the way) - Because RU stops all androgens and doesn't go systemic? My T was very high so need to block that on scalp as well i guess.
Oestrogel cream - for temples 17 B estradiol since temples are more estrogen dependent i presume? Also my estrogen is low as I said.

Other options -
Morr F
Topical minoxidil plus 0.1% dutasteride
Laser Cap - Tried 6 months don't think it worked, shed more on it.

Looking for stemoxydine instead of minoxidil since minoxidil is a long commitment and makes hair look like sh*t. Anyone else tried topical minoxidil plus duta? Wary of duta since want to go into finasteride first.

Lot of diffuse thinning on my head due to severe shedding the last few months. If I can stabilize hair loss maybe I can get a hair transplant in 5-6 months? Not sure how good my donor area is.

Added lots of pictures - dry and wet including donor area (hair is cut really short on the back).

Do you guys think I have any option other than a Hair transplant? Does it seem like fina+ru or other meds can restore my hair?

Any recommendations appreciated. Thanks!

You can try the meds you may be a good responder. But right now i think your donor looks weak and you probably arent a suitable transplant candidate. smp may be a better option looks pretty good on guys with dark skin & a beard.

SMP is crap imo. Not getting it, rather a hair system than going for smp. Do you think the back of head doesn't have enough donor hair?

Your donor looks succeptable to thinning. Meaning that even if you got a transplant the hair is likley to fall out as its sensitive to DHT. It may be worth going to see some reputable doctors in person to see what they think.

True but I have thyroid and had some nutrient deficiencies like very low ferritin (20) as well so that might have something to do with it.

You got a hair transplant too right? Are you on finasteride? Because docs ask most patients to get on finasteride right.
