I went to a friends party last week and two twins who I know also went, I haven't seen them for a few years, they are 25 years old, one thing that grabbed my attention was that one of the twins had diffused thinning, no hairline and was about NW5 and the other had only a slight recession with a very strong crown, about NW2, it just goes to show that even if you have an identicle twin theres no guarantee that hairloss will affect both in the same way.

if they're truly identical, then there's an awesome chance that the hairy one is on something to help their hair or got a transplant. identical twins share exactly the same DNA and genetic makeup if I'm remembering my biology class in college correctly, so if one of them balds, the other one will as well, unless they do something to stop it. Likewise, if one of them isn't balding naturally, then the other one won't either. That's why Propecia used some identical twins in their studies. Is it possible that the twins are fraternal but look a lot alike and you're mistaking them for identical? Because identical twins will have the same genes but fraternal twins will have some different genes.

Another thing to point out is that certain things can kickstart baldness early like steroid use or Accutane, just to name a couple things. It won't affect the final outcome but it might get your balding started earlier than your genetics would have otherwise. Basically I'm saying that environmental factors are relevant as well as genetic factors. Steroids won't make you bald if you don't have the genes-check out Jose Canseco as Michael Barry always states. But if you have androgenetic alopecia in your genes, they certainly won't help and could accelerate.

So maybe the worse off twin is just ahead of the other one. They should eventually catch up though if they are truly identical twins and they aren't on any medications or haven't had transplants, etc
