The jury is still out on 5AR inhibitors and prostate cancer; there are currently a few trials going on that indicate it may prevent cancers and reduce overall mortality, but it's still up in the air. DHT is a contributor to prostate cancer, and it makes sense that reducing that could quite possibly reduce prostate cancer incidence. However, as Finfighter said, it alters PSA levels making detection a bit more difficult.

The big finasteride prostate prevention trial showed a 30% reduction in prostate cancer with men taking finasteride.. shortly after, they determined that it also increases risk of more aggressive cancer, then even more research came out that showed this may not be the case.

It's still being considered as a potential treatment. As a person who has watched two family members struggle and succumb to prostate cancer, it would be fantastic news if finasteride/dutasteride could be used as a preventative measure.

When it comes down to it though, a young 20-something guy taking 1mg finasteride for hair loss already has an almost 0% risk of prostate cancer..
