Comebackkid: Basically, I think when you're taking a 5ar blocker like Finasteride
or Dutasteride you're reasonably well sorted against DHT activating the androgen receptor - and DHT is the most potent at doing so - but there are still other androgens that activate the AR like Testosterone and Androstenedione (which are the androgens I believe that lead to a more diffuse thinning pattern). So, your options to counteract the effects of these remaining androgens mostly lie with substances that block the AR completely. In my research these would include; topical Spironolactone, topical Flutamide, RU, Fluridil and perhaps Revivogen (as I think it's been loosely demonstrated this is effective at blocking the effects of Testosterone in the scalp - or something like that!).
Those who have the greatest success with Propecia are, I think, those who are less sensitive to androgens in general. If you're unlucky enough to be very sensitive to androgens then although Propecia may slow down your loss, I don't think you'd really have much chance of halting hair loss progression when your androgen receptors are continuing to be activated by other androgens. This is happening with the guys that are less sensitive to androgens too, but without as great an impact.
I'm not saying that this is the best way to do it, it's just my opinion. I think people need to do their own research and talk to others on hair loss forums as, invariably, what works for someone doesn't work for someone else! Hope that sort of helps.

mmm... we may be on to somthing here guys... now I NEED everyone that reads this tread to TRY to pitch there help in...
So... we DO know that RH can be caused by higher levels of T... however even though dht is reduced why do some people get diffuse thinning when they did not before...
So if those that have sensitive androgens recepters to T then it makes since that when one gets on propecia they get diffuse thinning..
So what can be done ?
Lets look at what CAN be done.... if one is STARTING to thin it may be because of dht.. BUT in people that have sensitive androgen recepters in there scalp react bad to the HIGH T levels then thats why diffuse thinning occurs in those people..
So its kinda shitty because either way your f***ed... your f***ed because dht is hurting you.. but WE DO KNOW.. that your even MORE f***ed with sensitive androgen recepters due to higher T...
so what can be done to get DHT levels down without rasing T levels because High T levels in those with senstive androgen recepters re-act to diffuse thinning...
All this makes logical since in those LIKE ME that have been diffusing while on propecia...
So is there anything that can be done to lower DHT without raising T levels...
Only thing I can think of is it's not possable from within the body...however it may be possaile to slow it down with spironolactone...which makes since...
Also can dutasteride be of any bennift on how it lowers DHT ??

so according to the mods sprio cream 5% will be sold agian this upcoming wensday... After doing research it seem that the cream version IS BETTER then the liquid...
So after reading all this I think my new program will be as follows:
Nizoral 1x a week leave in 2-3 mins ( really dries my hair way too much) But I follow with:
Vive Pro Thickening Shampoo and cond. to help.
therapro shampoo system every now and then
Propecia 0.25 mg ( 1x per day)
Sprio S5% cream 2x daily
Biosil 5mg ( 2x per day)
Biotin 5mg ( 1x per day)
Green tea pill ( 2x per day)
Omega-3 ( epa-dha)- 3x a day
I mean it makes logical since that some people react bad to propecia because of senstive androgen recepters re-act to diffuse thinning... I mean DHt is a cause but to takle it with lower a DHT bloker so that T levels dont spike up along with S5 cream may be the way to go.
