Definitely a poor design on the study. The lack of a control group really weakens the credibility of the results. The size of the treatment group (52) isn´t exactly impressive either..., but consistent results of all but 6 of them does point to a certain effect.

Though it´s really hard to evaluate the degree of those results without a control group.

I am 100% positive that spironolactone can get absorbed and have an impact systemically. After 2 days of applications (5% cream) my nipples start to burn and tingle. I have been on and off four different times and have had the same reaction - that is not a coincidence.

Did you stop use after that or can you give us a fallow up?
