I did a medline search from 1960 through 2004 and could not find one reference for the use of 2% ketoconazole cream for treating male pattern baldness. It seems to me that if the topical application of 2% nizoral to the scalp inhibits DHT +/- other androgens, the application of a 2% cream to the scalp (compared with the shampoo) would allow a greater amount of the nizoral to penetrate the scalp and maintain therapeutic concentrations for hours at a time, instead of minutes and therefore potentially have a greater effect.
