I have diffuse thinning. Is it normal for there to be red spots in thinning areas?

Yeah, high DHT is usually accompanied by sebhorric dermatitis/dandruff. It's not the cause of hair loss, but it often accompanies it. That's why the big three is finasteride, minoxidil and nizoral. The nizoral will reduce the red inflamed patches and dandruff.
