Help Me Interpret My Blood Test Results- Long Time finasteride User

Long time finasteride user here (10+ years) I'm 32 years old. Added dutasteride 3x a week about 6 months ago. So my regimen is dutasteride .5 mg 3x a week and finasteride 6.25mg 4x a week. Recently wasn't feeling good- lethargic fatigued low libido etc so I decided to get my hormone levels checked thru my doctor and was quite surprised by my results. Please if u could provide any input, I'd appreciate it. I've never tested any of these hormones outside of free and total testosterone which came back normal. Latest results below:

LH 3.6 mIU/mL range (1.7-8.6)
FSH 4.5 mIU/mL range (1.5-12.4)
Testosterone, free and total 831 ng/dL Range (264-916)
Free testosterone (direct) 9.4 pg/mL Range (8.7-25.1)
Dihydrotestosterone 5.3 ng/dL LOW. Range (30-85)
DHEA, serum 237 ng/dL. Range (31-701)
TSH 1.230 uIU/mL. Range (.450-4.50)
Prolactin 13.4 ng/mL Range (4.0-15.2)
Estrogens, total 114 pg/mL. Range (40-115)
SHBG, serum 80.3 nmol/L HIGH Range (16.5-55.9)

What concerns me the most:
Lower free test (direct)
Lower DHEA
Higher prolactin
Higher estrogens
Super high SHBG

I don’t have much to say, but with these results, I would think you would have no hair loss right progression right now. Is that true?

That's the effect of finasteride/dutasteride, it affects your hormones. It's known to raise estrogen. The long-term effect on hormones is unknown though.

How is your hair, why add dutasteride?

Any good resources to interpret what labs are optimal for hair loss prevention and regrowth. Similar scores not on Finasteride and still losing hair.

Your SHBG is really high and your Estradiol levels are high normal which could increase further. Your Free Testosterone, the important one is at the bottom of normal. Have you had cortisol levels checked? How is your liver fuction? Do you suffer from stress or depression and are you on any medications? This is very concerning and the perfect recipe for loss of libido, erectile dysfunction, and many other things. I would be concerned and get your doctors insight.

Thanks for the insight lacey. I have not had cortisol checked, what would this help to reveal? Liver function is normal based on blood tests done at the same time. I'm pretty active so these hormone levels r a bit eye opening. I was diagnosed with general anxiety and depression about 2 yrs ago. No other meds regularly but lots of supplements. My gp is referring me to an endocrinologist.

Btw is free test related to shbg? Meaning the higher the shbg is, the lower the free test will be since shbg renders the test not usable by tissues?

High Cortisol levels can lead to depression and lower Free Testosterone I believe. I am not 100% sure but would bring that up upon seeing your endocrinologist.

I'm wondering if I should drop dutasteride and stick to just finasteride. However I don't know how long I've been this way as I've never gotten these hormones tested so I have no baseline over the last 10+ years. I've only ever checked serum total test which came back high normal just like these results. Any suggestions?
