This case study presents the comprehensive hair restoration experience of Mr. Yash Joshi, encompassing his assessment, surgical procedure, recovery milestones, and outcome. The aim is to provide clear, transparent, and medically accurate information for anyone considering a hair transplant.

Patient Overview

  • Name: Yash Joshi
  • Age: 35 years
  • City: Vadodara, India
  • Diagnosis: Androgenetic Alopecia – Grade II
Mr. Yash had been experiencing progressive hair thinning for about 3–4 years. Over the last 2–3 years, he noticed a visible recession of the front hairline and early baldness in the frontal zone.

Family & Medical Background

  • Family history: Positive, his father also has male-pattern baldness, increasing his genetic risk.
  • General health: No history of hypertension, diabetes, asthma, or chronic illness.
  • Allergies: None known to food or medication.
  • Keloid tendency: Absent.
  • Previous treatments: Tried basic supplements and multivitamins, but without meaningful regrowth.

Consultation at Hairfree & Hairgrow Clinic

Mr. Yash met Dr. Jinkal Kunjadiya, dermatologist and hair transplant specialist, at the Surat branch.

A detailed scalp evaluation, including trichoscopy, was performed to understand the quality of donor hair and the extent of thinning.

After discussing the procedure, expected results, potential risks, and post-care requirements, the patient confidently opted for a hair transplant.

Treatment Summary

  • Date of Procedure: 17 June 2022
  • Technique Used: Big-FUE (Follicular Unit Extraction)
  • Total Grafts Implanted: 2608
  • Post-operative Care:
    • Five PRP sessions (every 2 months during the first year)
    • DHT-blocking medication
    • Topical Minoxidil
    • Nutritional supplements
This combination supports both transplanted and existing hair growth, helping maintain long-term results.

Step-Wise Hair Transplant Procedure

1. Planning & Hairline Design

A customized front hairline and temple outline were created based on the patient’s facial proportions and expectations. The mid-scalp was also marked to improve overall density.

  • Trichoscopy helped assess follicle health, miniaturization, and donor reserve.
  • A patch test confirmed he had no sensitivity to local anesthetics.
  • His scalp was then shaved and cleaned with an antiseptic solution.

2. Local Anesthesia

Modified block anesthesia was administered using a near-painless technique to ensure maximum comfort during the procedure.

3. Slit Creation

Slits were created using sapphire blades for better precision and the natural direction of hair growth:

  • 1.2 mm blade for the hairline
  • 1.4 mm blade for the remaining frontal and mid-scalp areas
Proper angulation and density planning at this stage are crucial for achieving a natural appearance.

4. Graft Extraction

Extraction was performed under magnification using a 0.8 mm serrated titanium punch, ensuring minimal trauma to the donor site.

Graft Quality

  • 20% single-hair grafts
  • 80% multi-hair grafts
  • A good number of 3-hair follicular units
  • Thick, dark, high-quality donor hair
Thin follicles from the lower donor zone were selected for the hairline for a softer, natural transition. Grafts were immediately stored in a chilled tray containing Gro-Factor solution to maintain vitality.

5. Graft Implantation

Implantation was carried out using K.E.E.P. implanters, which help place grafts at the correct depth and direction. The implantation phase took approximately 2 hours.

Post-Procedure Management

After the surgery, a dressing was applied, medications were provided, and detailed aftercare instructions were explained. Mr. Yash was given a direct contact number for medical support throughout his recovery.

Post-Transplant Recovery & Growth Timeline

1. Shedding Phase (0–3 months)

It is normal for 70–80% of implanted hairs to shed during this period. Two PRP sessions were performed to support healing and stabilize existing hair. By the end of 35 days, the donor area had healed well, and the density looked normal again by 2–3 months.

2. Early Growth Phase (3–4 months)

Fine “baby hairs” began to emerge gradually. A third PRP session boosted both transplanted and existing hair growth. By the fifth month, trimmed natural hair provided a visually fuller look.

3. Active Growth Phase (6–7 months)

This stage showed noticeable improvement in coverage. Approximately 80% of the transplanted follicles were actively growing.

Though the hair shafts varied in length, the patient was already satisfied with the density. A booster PRP session was given around the 9th month.

4. Final Results (10 months)

By the tenth month, Mr. Yash achieved his complete hair transplant result:

  • A natural, well-defined hairline
  • Healthy density across the front and mid-scalp
  • Multiple haircuts done successfully
  • Maintenance limited to minoxidil and optional PRP
He was extremely pleased with the outcome and shared high satisfaction with the procedure.
