First Consultation

When Didarul came in, the first thing I asked him was simple:

“How long have you been dealing with hair loss?”

He told me it had been progressing for several years and had now reached a stage where most of the front and mid-scalp was gone. Like many patients with advanced hair loss, he had already spent a lot of time researching online and speaking to different clinics.

Then I explained something important to him:

“At Grade 6, we’re not just restoring hair; we're managing limited resources.”

Understanding His Condition

In his case, the hair loss pattern was quite advanced:

  • The frontal area completely receded.
  • Mid-scalp significantly thinned
  • Crown starting to open up
So I asked him:

“Do you know what really matters most at this stage?”

Not a low hairline. Not maximum density everywhere.

It’s proper planning and smart use of grafts.

Donor Area Evaluation

We moved to the donor area, the back of the scalp.

This is where I always slow down and assess carefully:

  • Density
  • Hair thickness
  • Follicle quality
Because honestly, this decides everything.

In Didarul’s case, his donor area was stable and usable, which meant we could move forward safely.

Planning the Procedure

I explained the plan to him clearly:

  • Around 4500 grafts
  • Focus on the front and mid-scalp
  • Conservative, natural hairline
Then I told him something most patients don’t hear:

“We are not trying to give you teenage density. We are trying to give you a natural, believable result that will last.”

He understood that.

The Procedure

On the day of surgery, everything was done step by step.

  • Local anesthesia for comfort
  • Extraction using the FUE technique
  • Each graft handled carefully under magnification
  • Placement done following natural angles and direction
What matters most during implantation is not speed; it’s precision.

Early Phase (0–3 Months)

After the procedure, I had already prepared him for this phase.

“You will lose the transplanted hair initially; don't panic.”

And that’s exactly what happened.

  • Shedding phase started
  • Donor area healed well
  • No complications
This is where many patients get anxious, but this stage is completely normal.

Growth Phase (3–9 Months)

Around 3–4 months, new hair started appearing.

I told him:

“This is just the beginning; the hair will be thin at first.”

Gradually:

  • Hair started thickening
  • Density began improving
  • Overall appearance looked better month by month.

11-Month Result

Now at 11 months, the difference is clearly visible.

What I observe clinically:

  • Hairline looks natural and not forced
  • Good coverage in the frontal and mid-scalp
  • Density is balanced, not overdone.
  • Hair direction blends well with existing hair.
Most importantly, it does not look like a transplant.

Final Thoughts I Shared With Him

During his last follow-up, I told him:

“This is a good result for Grade 6, not because it’s dense, but because it looks natural.”

That’s always the goal.

What You Should Understand From This Case

If you’re at a similar stage, here’s what I would tell you directly:

  • Advanced baldness can be improved, but not overcorrected
  • Donor hair is limited; it must be used wisely.
  • A natural result is always better than an aggressive one.
  • Results take time, usually up to 12–15 months
