The follicles have been impaired (presumably by our old friend DHT) and so are unable to produce healthy sized hairs. Which means these hairs are so short they've never been cut - hence the thin, pointy top.
Depending on how your treatment is progressing this could be either; a sign that the process of miniaturization is ongoing, or; that after a finasteride/dutasteride/etc induced shed the follicles are recovering and in the process of growing gradually larger hairs. Impossible for me to say which it is.
