What are peoples actual expectations of this ? I'm too scared to take finasteride incase i wake up with a vagina and so the idea of a topical that reduces DHT on the scalp to the same amount the pill does but with very low systemic build up would be pretty ideal.

I imagine the clinical trials stating that there was barely any systemic build up, only for us to look further at the details to see that this was an observation they made only a couple days after application .. o_O

They've been testing this stuff for years, at one point they got it down to 1/3rd of the systematic effects of finasteride so they went down to the drawing boards to lower it again

I thought i read somewhere that they got systemic effects down by 15 times that of finasteride. Im not overly hopeful though - although at least it will most likely be the soonest news we receive regarding soon to be marketed treatments.

15 might've been after the 1/3rd wouldn't doubt it I'm sure there trying to keep it down as much as possible.

Any idea if this would still be effective if someone has been taking finasteride orally for years?

That's the big question, the point of such kind of product is to be as effective as a systemic product without being systemic itself
Biggest hint of this theory is that in their clinical trials, there's a finasteride group to compare results

Also they had molecule delivery problems, findind a good delivery protocol for 5AR1 to penetrate the skin and reach follicles and make this molecule degrade or go inactive once in the bloodstream

The only reason Polichem doesn't lower serum DHT as significantly as oral finasteride is because the trials have used a very, very, very small amount of the topical. They've been trying to work around that, but we won't really know until the latest data are in.

It will still give side effects, cause it still go systemic, may be less cause it go systemic less, but also less results since the amount of the dht reduced in the scalp is lower than with the oral dose .
So, for me, 0 expectation, I will not use it and I used for almost 20 years oral (finasteride then duta ) . There more, a lot more , exiting things on the horizons than this 3 decades old drug that may be, even arrive faster, like cb-03-01 or fevipiprant .

Is like , when Uphon released the foam version of minoxidil, is like " yeah yeah, great thing, now invent a time machine and release it 20 years before instead of the greasy version " .

What stops the systemic dht from floating up into the scalp and attaching to the follicles? The stuff is a topical 5ar2 inhibitor, not a dht receptor blocker...

f*** it. Either way, if its released ill buy it and use it and give it a good year to see what happens. If its a bust then hopefully at the end of that year another new product may be out and ill put my hope in it.
