I just assumed it was concealers because his crown was bare on a location film but when they went back to the studio live it was covered, (like J Clarkson now on Top Gear who has also started using it in the studio but location films are bare?) Possible media seeing Nano and immediately assuming it's a hair transplant?
Much like the idiots in this well known daily who suspected a piece, within 2 hours of top left pic. It's obviously Nano/Toppik whatever

No Derren admitted to it in an interview he's pretty open about it, he also came out as gay in the press a few years back.
I'd guess its a combo of hair transplant, concealers and he's probably using meds aswell.
As for Clarkson you could be right but I also think alot of people mix up how old the repeats of Top gear are, his hair has deteriorated over the years from a curly bush to pretty thin and a bald crown these days. However I dont think it really bothers him as he's fat and ugly with Tombstone teeth and still manages to get young women to have affairs with him. (if the papers are to be believed).

Re Clarkson, It's only been the last series of Top Gear which has finished now.
He only used it in the studio because the recent films they did and showed in the last series he had the bald crown, but back to studio he was definitely using fibres as I don't think he's a hair transplant type of guy
