Concerned that I might have MORE than male pattern baldness causing hairloss

Basically, I'm nearly 7 months into using Propecia and Nizoral. I had diffuse-thinning hair with a NW2 pattern before, but it's MUCH worse now, basically I went from diffusing in a 'lightning bolt' pattern on top, and easily disguisable (not noticeable) to me having a solar panel. It's affected me so much that I can't face being anywhere public without a hat, the hairloss is now so visible under and strong lights, I've lost all my social life, can't even face seeing family members. I've tried getting a buzzcut to hide it, but that doesn't disguise it, rather emphasises it, and makes my awful hairline look more apparent. I'm having issues with my girlfriend too as she denies I've got male pattern baldness and talks about 'naturally thin hair'.

I did shed a lot to start with. I'll admit my hair wasn't perfect before starting treatment, but it's a lot worse now, and I have no idea how to find out what, if anything, it might be other than male pattern baldness. The pattern seems classic male pattern baldness as far as I can tell, but there might be something stacked on top of it, or exacerbating it. I'm wondering if Propecia even works at all for me.

Iron deficiency can be ruled out as I was OK when checked out in January for suspected anaemia. Not sure what other possible causes it could be. I take Lorazepam for anxiety issues and preventative asthma inhalers, as far as my other medication goes. I also suffer from IBS and rhinitis, so I'm prone to inflammation based ailments. Any ideas what could be going on?

A dermatologist (a private, non NHS one) diagnosed Telogen Effluvium when I saw him in November. And male pattern baldness too of course, so there was something.

(Look at the old August 2011 pic I posted BTW - found photo used to make the ID card it's from, and it shows how amazingly thick my hair was not all that far back ago! If i had a scanner I'd...)
