4 Derms and no results!! Diffuse Hair Loss, flakes and sebum (Pics) no results on finasteride

Hi there! First of all I want to apologize because of my bad english. It is not my mother tongue so I will try to explain my story the best I can. :)

-My story:
I have been loosing hair since I stopped my treatment with roaccutan (isotretinoin) for my acne. After stopped with this drug my hair began to fall slowly in a diffuse pattern all over my scalp. This wasn't noticeable until a year ago. This hair loss came together with an increased sebum production, as well as pain and itchy scalp. I could feel this pain in the areas where my hair loss was more evident (sides, above my ears, frontal and crown). Those symptoms became more appreciable when I started my treatment with minoxidil 5% (but i will tell you this later). I started losing hair when I was 17 years old. I must confess that i drank alcohol when I was in roaccutan but the blood tests never showed bad results. Anyway, now I am 23 years old and my hair loss has became more noticeable than ever.

-My treatments:
I had appointments with 4 different dermatologists (no one seem to be very interested in my hair loss). Two public and two private. Firstly, I was prescribed on minoxidil 5%. This didn't gave me any result and I was prescribed on finasteride 2mg and ketoconazole for my scalp problems. Due to the alcohol or the propyleneglycol I stopped applying minoxidil and continued with finasteride. My hair situation never improved and I think it got worse in terms of density. I was on finasteride for 2 or 3 years aprox and I gave up few months ago as I didn't see any improvement. I switched then to avodart (dutas 0'5). I were two months combining finas with dutas every other day and I started to take dutas everyday on January. I had a massive shedding and I lost 30% of my remaining hair. I continued with this shedding for 5 or 6 months and nowadays I can see new hairs growing in the front. However I am sure those hairs growing are the same I lost before so I could say this is not helping me in recover the hair I lost through those years.

- My hypothesis:
I had too much time to think about this. It became an obsession for me and I can't stop thinking how I can have such an agressive hair loss if there is no hair loss background in my family. My fahter, mother, uncles, grandfathers and grandmothers had a head full of hair. And, why I have this uncommon hair loss pattern? I loss my hair all over the scalp, also above my ears and sides of my nape.. That's weird! That's why I have been thinking in different hypothesis such as this hair loss could have been induced by the roaccutan (I heard lot of cases of permanent hair loss after stopping isotretinoin treatments due to an hypervitaminosis, excess of vitamin A hosted in the liver or the colon). One thing is clear for me: my sebum production, if it is not the cause it is an aggravating agent or a consequence of something happening inside of me. I have been thinking in folliculitis or scarring alopecia too (inflammation of hair follicles - I dont' know if sebum has something to do in this type of hair loss). I also have sebaceous cysts in my scalp (or something similar) and they are located in the areas where the pain and the itchy feelings are more noticeable.

Stay on finasteride and get a hairtransplant.
Hi M.G, thanks for answer. I was wondering why should I stay on finasteride if I didn't see any results with it. To be honest, I was much better before finasteride's shedding than after two years on it. On the other hand, I am a 22 y.o student so I don't have enough money for a hair transplant.

@M.G: He's on dutasteride whatsoever, so why you've advised him to stay on finasteride? dutasteride is the more potent drug, it should be enough. @behindemusgo Do you have dandruff? Try some good shampoos to get rid of the itchy scalp. Ketoconazole based shampoos never helped me... so I've picked piroctone olamine/ciclopirox olamine/tar based shampoos. I do alternate them...

I have seen 2 dermatologists. All they could suggest me was minoxidil. That's crazy. I don't understand why people go to dermatologists. There is another kind of specialists - trichologists. I guess they are more interested in hair. Why don't you see some trichologist then?

Well, trichology is still a branch of dermatology, so there's nothing bad to go to a <good> dermatologist. In the country where I live... I haven't seen a single doctor who says that he's trichologist.

I live in Madrid, Spain, and I think it is pretty uncommon to find any good trichologist if you don't pay 150 euros. I found a trichology departament in a public hospital so I will ask my doctor if I can have an appointment with them.

Those pics were taken this morning when I woke up. I showered my hair before go to sleep and I woke up like this. It may seem like my hair is wet but this is just the sebum my scalp produce every night. As I wrote, this problem began when I stopped my isotretinoin treatment (roaccutan) and it became bigger through the years.

Does it look like an male pattern baldness? In case the answer is yes, is it possible that another problem were aggravating my male pattern baldness?
My hairline is not receding and I am losing lot of hair everyday despite I am in my 5th month with dutasteride every day.

DUPA, Telogen Effluvium, whatever else makes you lose hair everywhere. Typically you don't lose on the sides which you seem to be experiencing.

I am thinking my hair loss and scalp problems have been triggered after my accutane tratment. I have seen lots of permanent hair loss after taking isotretinoin and in the pics looks exactly like my hair loss pattern. Maybe there is accutane stored in my liver (hypervitaminosis a) and I need to detox, maybe I have developed a super aggressive DUPA because of this medication (I mean it made my follicles more senstive to DHT or it accelerated my alopecia), or maybe i developed any kind of auto immune desease... Who knows. I will go to one of the best trichologist in Madrid and I will post whatever he tells me in order to help those ones which are experiencing the same as me. I will keep you updated! Thanks for your time guys!
